INTISARI
ALatar belakng: nemia merupakan salah satu
komplikasi tersering dari penyakit ginjal kronis (PGK), serta dapat
meningkatkan risiko mortalitas pada PGK hingga 52%. Terapi anemia dengan recombinant
human erythropoietin (rHuEPO) mampu meningkatkan kadar hemoglobin (Hb)
dengan sifat dose dependent. Dosis inisial 150 IU/kgBB/minggu
direkomendasikan oleh Kidney Disease Improving Global Outcome (KDIGO).
Belum ada studi mengenai hubungan dosis terapi rHuEPO terhadap perbaikan
kondisi anemia pada anak di Indonesia.
Tujuan: Mengetahui hubungan dosis terapi rHuEPO dengan kadar Hb dan kebutuhan transfusi darah pasien anak dengan anemia pada PGK.
Metode: Dilakukan studi kohort retrospektif dengan subjek pasien anak usia 2 – 18 tahun dengan anemia dan PGK di RSUP Dr. Sardjito periode Januari 2019 – Januari 2023. Sampel diambil secara consecutive sampling. Pengolahan data menggunakan perangkat lunak program pengolah data yang terkomputerisasi. Kurva ROC digunakan untuk menghitung cut-off, kemudian dilakukan analisis bivariat dan analisis multivariat. Hubungan antar variabel dinyatakan dengan risk ratio (RR) dan interval kepercayaan 95?ngan tingkat kemaknaan statistik p <0>
Hasil: Empat belas (23%) dari 61 total sampel yang
mendapat dosis rHuEPO optimal sesuai rekomendasi KDIGO. Berdasarkan kurva ROC,
didapatkan ambang batas dosis rHuEPO pada sampel
penelitian 76 IU/kgBB/minggu
untuk mencapai target Hb >10 g/dL. Dosis ?76 IU/kgBB/minggu dapat meningkatkan pencapaian target Hb > 10 g/dL
dibandingkan dengan dosis yang lebih
rendah (RR 2,02; IK 95%: 1,19-3,42; p 0,007). Dosis ?76 IU/kgBB/minggu tidak signifikan dalam menurunkan kebutuhan transfusi
darah dibandingkan dosis yang lebih rendah (RR 0,3; IK 95%
0,08-1,46; p>0,05).
Kesimpulan: Dosis rHuEPO ?76 IU/kgBB/minggu dapat meningkatkan pencapaian target hemoglobin, tetapi tidak signifikan dalam menurunkan kebutuhan transfusi darah pasien anak anemia dengan PGK di RSUP Dr. Sardjito.

ABSTRACT
Background:
Anemia is one of the most common complications of chronic kidney disease (CKD)
and increase the risk of mortality in CKD up to 52%. The effect of Recombinant
Human Erythropoietin (rHuEPO) in increasing hemoglobin level (Hb) are
dose-dependent. An initial dose of 150 IU/kg/week is recommended by Kidney
Disease Improving Global Outcome (KDIGO). There are no studies regarding the
effect of rHuEPO doses and improvement of anemia in children in Indonesia.
Objective:
To determine the effect of rHuEPO dose variations in achieving Hb target and
decrease blood transfusions in pediatric patients with anemia in CKD.
Methods: A retrospective cohort study was conducted with pediatric patients aged 2-18 years with anemia and CKD at RSUP Dr. Sardjito from January 2019 to January 2023. Samples were taken with consecutive sampling method. Data processing using computerized statistical software. The ROC curve used to calculate rHuEPO dose threshold, then bivariate analysis and multivariate analysis were performed. Results:
Fourteen (23%) of 61 total samples received the optimal dose of rHuEPO as
recommended by KDIGO. The rHuEPO dose threshold from ROC curve was 76
IU/kgBW/week to achieve Hb target >10 g/dL. Doses of ?76
IU/kgBW/week could increase achievement of Hb target >10 g/dL compared to
lower doses (RR 2.02; 95% CI: 1.19-3.42; p 0.007). Doses of ?76
IU/kg/week were not significant in decreasing blood transfusions demand
compared to lower doses (RR 0.3; 95% CI 0.08-1.46; p>0.05).
Conclusion: Doses of rHuEPO ?76 IU/kgW/week can increase the achievement of Hb target, but not significant in decreasing blood transfusions in anemic children with CKD at RSUP Dr. Sardjito.
