Skor Systemic Inflammation Response Index (SIRI) sebagai Faktor Kesintasan 1 Tahun Pada Karsinoma Sel Hati di RSUp Dr. Sardjito Yogyakarta

Latar Belakang: Pasien yang terdiagnosis KSH memiliki latar belakang respon inflamasi terhadap tumor padat yang terdapat pada hati, sehingga memicu reaksi inflamasi sistemik pertahanan tubuh terhadap pertumbuhan tumor tersebut. Skoring SIRI merupakan parameter yang hanya membutuhkan panel darah neutrofil, monosit, dan limfosit untuk menilai respon inflamasi pada tubuh. Saat ini belum ada penelitian yang menghitung skoring SIRI sebagai faktor kesintasan di RSUP Dr. Sardjito. Tujuan: Mengetahui nilai cut off Skor SIRI dan menilai tingginya nilai SIRI sebagai faktor kesintasan 1 tahun pasien KSH di RSUP Dr. Sardjito Yogyakarta. Metode: Penelitian dilakukan secara retrospektif, melibatkan 106 pasien yang terdiagnosa KSH pada tahun 2018-2022 di RSUP Dr. Sardjito. Analisis ROC digunakan untuk mendapatkan cut off nilai SIRI dengan metode youden index. Analisa statistik bivariat menggunakan uji Chi Square, Fischer Exact, hasil dinyatakan bermakna secara statistik bila nilai p < 0 p=0,001;>3,24 (p= 0,0001; CI 95% CI 1,846- 4,994) dan tindakan TACE (p= 0,009; CI 95% 0,24-0,814). Kesimpulan: Tingginya nilai SIRI dengan nilai cut off 3,24 merupakan faktor kesintasan 1 tahun pasien KSH di RSUP Dr. Sardjito.

Background: Patients diagnosed with Hepatocelluler Carcinoma have a background of inflammatory response to a solid tumor in the liver, thus triggering a systemic inflammation response of the bodys defence against the growth of the tumor. SIRI scores are a parameter that only requires blood panels of neutrophils, monocytes, and lymphocytes to evaluate the inflammatory response in the body. Currently, there is no study that calculates SIRI scores as a survival factor for Hepatocelluler Carcinoma in Dr. Sardjito Hospital. Objective: To calculate the cut off value of SIRI Score and to analyze the high SIRI score as a factor of 1 year survival of Hepatocelluler Carcinoma patients in RSUP Dr. Sardjito Yogyakarta. Methods: The study was conducted retrospectively, involving 106 patients who were diagnosed with Hepatocelluler Carcinoma in 2018-2022 at RSUP Dr. Sardjito. ROC analysis is used to get cut off SIRI values with the youden index method. Bivariate statistical analysis using Chi Square test, Fischer Exact, stated statistically meaningful results when p value < 0 p=0,001;>3,24(p= 0,0001; 85 % CI 1,846-4,994) and TACE (p = 0,009; 95 % CI 0.24-0,814). Conclussion: The high score SIRI with cut off point 3,24 is a 1-year survival factor for Hepatocelluler carcinoma patients in RSUP Dr. Sardjito.
