STRATEGI PEMERINTAH DESA DENGAN BERBAGAI TINGKAT CAKUPAN PESERTA DALAM MENCAPAI UNIVERSAL HEALTH COVERAGE (UHC) DI KABUPATEN KARIMUN

Latar Belakang : UHC merupakan salah satu target SDGs yang ditetapkan oleh PBB. Kemudian pemerintah mengeluarkan Inpres No. 1 Tahun 2022  tentang Optimalisasi Pelaksanaan Program JKN. Cakupan peserta JKN di Kabupaten Karimun 83,28%.

Tujuan : Untuk mengeksplorasi strategi pemerintah desa dengan berbagai tingkat cakupan peserta dalam mencapai UHC di Kabupaten Karimun.

Metode Penelitian : Metode kualitatif dengan pendekatan deskriptif analitis. Data dikumpulkan melalui Diskusi Kelompok Terarah, wawancara mendalam dan studi pustaka. Subyek penelitian adalah pemerintah desa dengan capaian UHC tertinggi, sedang dan terendah di area Pulau Karimun Kabupaten Karimun.

Hasil : Strategi pemerintah desa dalam mencapai UHC meliputi pendataan dan identifikasi individu yang tidak tercakup JKN, melibatkan partisipasi aktif kader Pusat Pelayanan Terpadu (Posyandu), Posbindu dan bidan desa; memberikan edukasi dan penyuluhan, mengusulkan JKN bagi masyarakat miskin kepada pemerintah daerah, dan bekerja sama dengan kantor kecamatan dan Dinas Kesehatan dalam pengusulan PBI. Implementasi strategi ini terkendala oleh belum adanya peraturan mengenai penggunaan dana desa untuk pendaftaran JKN, keterbatasan dana, dan penganggaran JKN membutuhkan dana yang besar. Sementara itu, tantangan dalam pelaksanaan program ini antara lain kekhawatiran tidak mampu membayar iuran JKN, kecenderungan menggunakan cara sendiri dalam pembiayaan kesehatan, rendahnya kesadaran akan pentingnya JKN dan kemungkinan terjadinya kecemburuan sosial atas pendaftaran peserta JKN.

Kesimpulan : Terdapat kesenjangan antara strategi pemerintah desa UHC tertinggi dan rendah diantaranya pemerintah desa UHC tertinggi melibatkan partisipasi aktif kader dari masyarakat.

Background: The government issued Presidential Instruction No. 1 of 2022 concerning Optimizing the Implementation of the JKN Program. Coverage of JKN participants in Karimun Regency is 83.28%.

Objective: To explore village government strategies with various levels of participant coverage in achieving UHC in Karimun Regency.

Resech methodology: This research uses a qualitative method with a descriptive-analytical approach. Data was collected through Focus Group Discussions, in-depth interviews, and literature reviews. The research subjects were village governments with the highest, medium, and lowest UHC achievements on Karimun Island in Karimun Regency.

Result: The village government's strategy in achieving UHC included data collection and identification of individuals who JKN does not cover, actively involving the participation of cadres of the Integrated Healthcare Center (Posyandu), Posbindu and village midwives; providing education and advice, proposing JKN for poor society to the local government, and collaborating with sub-district offices and the Health Office in proposing PBIs. Implementation of this strategy is constrained by the absence of regulations regarding the use of village funds for JKN registration, limited funds, and JKN budgeting requires significant funds. Meanwhile, challenges in implementing this program include worry about not being able to pay JKN contributions, tendency to use out-of-pocket methods in health financing, lack of awareness regarding the importance of JKN and the possibility of social jealousy over the registration of one of villagers.

Conclusion: There is a gap between the highest and lowest UHC village government strategies, including the active involvement of community cadres in the highest UHC village government.
