Dilema Etik Dalam Pengambilan Keputusan Klinis Terhadap Pasien Covid-19 Saat Terjadi Keterbatasan Oksigen Pada Masa Pandemi Varian Delta: Perspektif Dokter Jaga IGD

Pendahuluan: Covid-19 varian delta menyebabkan keterbatasan oksigen. Kondisi tersebut menyebabkan dokter jaga IGD mengalami dilema etik saat menghadapi pasien Covid-19. Tujuan penelitian ini adalah untuk mengetahui dilema etik yang dihadapi oleh dokter jaga IGD, mengetahui landasan prinsip etik dan proses pengambilan keputusan oleh dokter jaga IGD pada saat varian delta pandemi Covid-19. 

Metode: Penelitian menggunakan metode kualitatif-fenomenologi di dua rumah sakit di Jember sejak November 2022 sampai Februari 2023. Informan penelitian dipilih melalui metode purposive sampling dan didapatkan 9 orang dokter jaga IGD. Pengumpulan data dilakukan dengan metode wawancara mendalam. Instrumen penelitian utama adalah peneliti dan dibantu dengan catatan dan alat perekam. Analisis data dilakukan dengan metode Colaizzi.

Hasil : Didapatkan empat tema, yaitu: 1) Kondisi keterbatasan oksigen saat pandemi Covid-19; 2) Dilema etika Covid-19; 3) Landasan prinsip etik, dan 4) Proses pengambilan keputusan. Kategori yang didapatkan dari tema pertama antara lain: 1) Kondisi IGD; 2) Kondisi pasien; 3) Ketersediaan Oksigen; 4) Kondisi tenaga kesehatan; 5) Kekacauan. Kategori yang didapatkan dari dilema etika Covid-19 adalah definisi dilema, perasaan, dan strategi. Kategori yang didapatkan dari tema landasan prinsip etik adalah: 1) Respect for autonomy; 2) Beneficence; 3) Non maleficence; 4) Justice. Kategori yang didapatkan dari tema proses pengambilan keputusan, yaitu: 1) Pertimbangan klinis dan kualitas hidup; 2) Pertimbangan pasien, keluarga dan masyarakat, dan 3) Pertimbangan kebijakan.

Kesimpulan: Dilema etika yang dihadapi dokter sangat beragam. Empat Prinsip dasar etik dijadikan landasan oleh dokter dalam melakukan pelayanan. Pertimbangan dokter dalam pengambilan keputusan adalah: 1) kondisi klinis dan prognosis; 2) preferensi pasien dan keluarga; 3) kebijakan. Pelaksanaan prinsip etik oleh informan kurang sesuai teori dan tidak ada informan yang mengetahui teori penyelesaian dilema seperti four boxes method, dan framework oleh WRHA.

Introduction: Covid-19 delta variant have caused oxygen shortage. This condition causes the emergency doctor to experience an ethical dilemma when dealing with Covid-19 patients. The purpose of this study was to find out the ethical dilemmas faced by the emergency doctors, knowing the basic principles of ethics and the decision-making process by the emergency doctors during the delta varian of the Covid-19 pandemic.

Methods: The study used a qualitative-phenomenological method in two hospitals in Jember from November 2022 to February 2023. 9 research’s informants were selected through a purposive sampling method. Data collection was carried out by in-depth interview method. The main research instrument is the researcher. Data analysis was using the Colaizzi’s method.

Results: Four themes were obtained: 1) The condition of limited oxygen during the Covid-19 pandemic; 2) The ethical dilemma of Covid-19; 3) The ethical principles, and 4) The decision-making process. The categories obtained from the first theme include: 1) Emergency Room Conditions; 2) Condition of the patient; 3) Availability of Oxygen; 4) health workers’s condition 5) Chaos. The categories derived from the ethical dilemmas are definitions of dilemmas, feelings, and strategies. The categories obtained from the theme of ethical principles are: 1) Respect for autonomy; 2) Beneficence; 3) Non maleficence; 4) Justice. The categories obtained from the theme of the decision-making process are: 1) Clinical considerations and quality of life; 2) Considerations of patients and families and 3) Policy considerations.

Conclusion: The ethical dilemmas faced by emergency doctors are varied. The four basic principles of ethics are used by doctors in providing health services. The doctor's considerations in making a decision are: 1) clinical condition and patient's prognosis; 2) patient and family preferences; 3) policy. None of the informants knew the theory of solving dilemmas.
