Hubungan NLR (Neutrophil Lymphocyte Ratio) Dengan Los (Length Of Stay) Perawatan Intensif Pada Pasien Pediatrik Yang Menjalani Pembiusan Di Masa Pandemi Covid-19

Latar Belakang: Virus Covid 19 dapat menyebabkan perburukan kondisi yang cepat dan membutuhkan perawatan intensif di ruang khusus. Ruang perawatan intensif pada kondisi normal masih kurang, dan dengan adanya covid 19 ini semakin membuat jumlah ruang perawatan intensif berkurang karena dibutuhkan ruangan yang khusus. Perlu adanya suatu parameter yang mudah dan cepat untuk memprediksi lama rawat di ICU agar penanganan dapat lebih terfokus dan ketersediaan ICU jadi lebih baik. Tujuan: Mengetahui Hubungan NLR (Neutrophile Lymphocyte Ratio) dengan LOS (Length of Stay) perawatan intensif paska operasi pada pasien pediatrik yang menjalani pembiusan di masa pandemi Covid-19.Metode Penelitian: Penelitian ini dilakukan secara retrospektif pada bulan Maret 2020 – April 2021 dengan mengambil sampel dari ICU RSUP Dr Sardjito dengan Kriteria inklusi penelitian ini adalah pasien dengan usia dibawah 18 tahun dengan kriteria eksklusi yaitu pasien yang terdiagnosis dengan imuno-related disease dan riwayat kemoterapi, serta pasien yang pindah rawat dari ICU RSUP Dr Sardjito Yogyakarta. Hubungan NLR dan LOS dinilai menggunakan statistik dengan software SPSS dengan sebelumnya menentukan cut-off NLR yang diuji dengan mempertimbangkan hasil tes ROC. Cut-off LOS diambil dari mean atau median dari sampel. Kemudian hubungan NLR dan LOS diuji menggunakan uji univariabel dan hubungan dengan variabel luar diuji menggunakan uji multivariabel.Hasil: Dari 173 pasien yang diteliti didapatkan hubungan yang bermakna antara NLR post operasi dengan LOS dengan nilai p : 0,000 dan OR 3,4. Untuk NLR pre operasi tidak didapatkan hubungan yang bermakna dengan nilai p : 0,087, sedangkan untuk faktor-faktor yang bermakna mempengaruhi LOS selain NLR pre dan post operasi didapatkan variabel usia 0-2 tahun dengan nilai p : 0,041 dengan OR 3,2.Kesimpulan: NLR post operasi berhubungan positif dengan memanjangnya Length of Stay (LOS) pada pasien pediatrik yang menjalani pembiusan di masa pandemi.

Background: The Covid 19 virus can cause a rapid worsening of conditions and requires intensive care in a special room. Intensive care rooms under normal conditions are still lacking, and with the presence of Covid-19 this has further reduced the number of intensive care rooms because a special room is needed and cannot be used for other types of diseases. An easy and fast parameter is needed to predict the length of stay in the ICU so that treatment can be more focused and ICU availability better. Objective: Knowing the Relationship between NLR (Neutrophile Lymphocyte Ratio) and LOS (Length of Stay) postoperative intensive care in pediatric patients undergoing anesthesia during the Covid-19 pandemic. Research Methods: This study was conducted retrospectively from March 2020 – April 2021 by taking samples from the ICU of Dr. Sardjito General Hospital. The inclusion criteria for this study were patients under 18 years of age with exclusion criteria, namely patients diagnosed with an immuno-related disease and a history of chemotherapy, as well as patients that was transfered from the ICU Dr. Sardjito Hospital, Yogyakarta. The relationship between NLR and LOS was assessed using statistics with SPSS software by previously determining the cut-off NLR tested by considering the results of the ROC test. Cut-off LOS is taken from the mean or median of the sample. Then the relationship between NLR and LOS was tested using a univariable test and the relationship with external variables was tested using a multivariable test. Results: Of the 173 patients studied, a significant association was found between postoperative NLR and LOS with a p value of 0.000 and an OR of 3.4. For preoperative NLR, there was no significant relationship with p value: 0.087, whereas for factors that significantly affected LOS other than pre and postoperative NLR, the age variable 0-2 years was obtained with p value: 0.041 with OR 3.2. Conclusion: Postoperative NLR is positively associated with lengthening the Length of Stay (LOS) in pediatric patients undergoing anesthesia during a pandemic.
