Analisis Biaya dan Kesesuaian Biaya Riil Terhadap Tarif INA-CBGs Pasien Penyakit Paru Obstruktif Kronis Peserta JKN Rawat Jalan di Rumah Sakit Akademik Universitas Gadjah Mada

Terapi untuk penyakit Paru Obstruktif Kronis
(PPOK) memerlukan biaya yang tinggi. INA-CBGs adalah sistem paket pembayaran
kesehatan yang didasarkan pada prosedur yang diterima pasien dan diagnosis
akhir yang diselenggarakan oleh Badan Penyelenggara Jaminan Sosial (BPJS).
Dalam penerapannya, tidak jarang ditemukan perbedaan antara biaya riil dengan
tarif INA-CBGs. Penelitian ini bertujuan untuk mengetahui rata-rata biaya
terapi PPOK dan komponen biaya penyusunnya, faktor yang mempengaruhi, serta
kesesuaian biaya riil dengan tarif INA-CBGs di RSA UGM Yogyakarta.
Penelitian ini merupakan penelitian analitik
non-eksperimental dengan rancangan cross
sectional. Pegambilan data dilakukan secara retrospektif melalui catatan
medis pasien dan rincian biaya perawatan pasien. Subjek dalam penelitian ini
adalah 98 pasien JKN PPOK dengan 170 episode rawat
jalan pada periode
Oktober-Desember 2022. Perspektif yang digunakan yaitu perspektif rumah sakit
sehingga biaya yang dihitung adalah biaya medis langsung. Pengolahan dan
analisis data dilakukan menggunakan analisis Kruskal-Walls, Mann-Whitney, dan One way ANOVA untuk mengetahui faktor
yang mempengaruhi biaya terapi serta Wilcoxon signed ranks test dan one sample t-test untuk mengetahui kesesuaian biaya riil dengan tarif INA-CBGs.
Hasil penelitian menunjukkan rata-rata biaya terapi per episode dengan 170 episode pasien PPOK rawat jalan sebesar Rp526.148,68 ± 342.278,60 dengan komponen biaya yang paling dominan adalah biaya obat (78,03%). Usia, jenis kelamin, komorbid, dan lama menderita tidak mempengaruhi biaya terapi pasien PPOK (p>0,005). Total biaya riil lebih tinggi dibandingkan dengan total tarif INA-CBGs dengan selisih sebesar Rp54.819.476,00. Hasil uji statistika menunjukkan terdapat perbedaan bermakna antara biaya riil dengan semua kelompok dengan kode INA-CBGs yaitu Q-5-44-0, J-3-13-0, dan J-3-16-0 (p<0>
Therapy for
Chronic Obstructive Pulmonary Disease (COPD) requires a high
cost. INA-CBGs is a package-health payment system based on procedures received
by patients and final diagnoses organized by the Social Security Agency on Health
(BPJS). In practice, it is not uncommon to find there is a
difference
between the real cost and the INA-CBGs’ tariff. This
study aims to determine the average cost of therapy for COPD and its component,
influencing factors, and the suitability of real cost with the INA-CBGs’ tariff at
RSA UGM Yogyakarta.
This research is
categorized as non-experimental analytic study with a cross sectional design.
Data collection was carried out retrospectively through patient medical records
and details of patient care cost. The subjects in this study were 98 patients of COPD which also JKN participants with 170 episodes of hospitalization from
October-December 2022. The perspective used is the hospital perspective so that
the cost calculated as direct medical cost. Data analysis was done
using Kruskal-Walls, Mann-Whitney, and One way ANOVA analysis to determine influencing factors, meanwhile Wilcoxon signed ranks test and one
sample t-test to determine the comparison between INA-CBGs’
tariff and the real cost.
The
results shows that the average cost of therapy per episode with 170 episodes for COPD is
Rp526,148.68 ± 342,278.60. The most dominant component is the cost of drugs
(78.03%). Age, gender, comorbidities, and length of suffering did not affect
the cost of therapy (p>0.005). The total of the real
cost is higher than the total INA-CBGs’ tariff
with a difference of Rp54,819,476.00. The statistical test
results shows a significant difference between the real costs and all groups
with the INA-CBGs’ tariff code Q-5-44-0, J-3-13-0, and J-3-16-0
(p<0> Evaluation of the real cost and INA-CBGs’ tariff needs
to be carried out by the hospital and the BPJS.
