Evaluasi Penggunaan Obat Antihipertensi dan Antidiabetes pada Pasien Gagal Ginjal Kronis dengan Hemodialisis Rutin di RSUP dr. Soeradji Tirtonegoro Klaten

Pasien
gagal ginjal kronis yang berada pada tingkat 5 atau end stage kidney disease
(ESKD) memerlukan terapi hemodialisis untuk membersihkan sisa metabolit dan
kelebihan air pada tubuh. Proses hemodialisis pada pasien mengakibatkan risiko efek
samping, seperti hipotensi dan hipoglikemia. Penelitian ini bertujuan untuk
mengetahui profil penggunaan obat antihipertensi dan antidiabetes, meliputi
waktu penggunaan sebelum atau sesudah hemodialisis terhadap luaran terapi
tekanan darah dan kadar glukosa darah pasien selama hemodialisis.
Penelitian
dilakukan menggunakan metode cross sectional. Pengambilan data dilakukan
secara prospektif pada Februari 2023. Pengambilan sampel dilakukan secara purposive
sampling sebanyak 125 sesi hemodialisis pada 49 pasien di Instalasi Hemodialisis
RSUP dr. Soeradji Tirtonegoro Klaten. Sampel dipilih dengan kriteria inklusi,
yaitu berumur ?18 tahun, melakukan hemodialisis
rutin, mengonsumsi obat antihipertensi atau antidiabetes, dan bersedia untuk
diwawancara. Analisis data dilakukan secara deskriptif berdasarkan data dari
rekam medis dan hasil wawancara dengan pasien.
Hasil penelitian menunjukkan konsumsi obat antihipertensi dilakukan sebelum hemodialisis oleh semua sampel, sedangkan obat antidiabetes diberikan sebelum HD pada 15 pasien (62,5%). Penurunan tekanan darah antara sebelum dengan sesudah HD terjadi pada 76% sesi hemodialisis dengan 1 sesi pasien mengalami hipotensi TD sistolik <100>
Patient
with chronic kidney disease stage 5 or end stage kidney disease (ESKD) require
hemodialysis therapy to clean up the residue of metabolites and excess water in
the body. Hemodialysis process in patient causes risk of side effects, such as
hypotension or hypoglycemia. This study aims to determine profile of the use of
antihypertensive and antidiabetic drugs, including the time of use before or
after hemodialysis to the outcome of blood pressure and blood glucose level on
patient during hemodialysis.
The
study using a prospective cross-sectional method in February 2023. Sampling by
purposive sampling of 125 hemodialysis sessions in 49 patients at Hemodialysis
Installation RSUP dr. Soeradji Tirtonegoro Klaten. Samples were selected with
inclusion criteria, <=18 years old, undergoing hemodialysis routine, taking
antihypertensive or/and antidiabetic drugs, and being willing to be
interviewed. Data analysis was carried out descriptively based on data from
medical records and the results of interview with patients.
The result showed that the
consumption of antihypertensive were before hemodialysis by all samples, while
some antidiabetic drugs were given before HD by 15 patients (62,5%). Decreased
blood pressure between before and after HD occurred in 76% sampels. Random
blood sugar levels after HD <70>200 mg/dL. The majority of sampels showed symptoms of
dizziness, nausea, muscle cramps, and no symptoms of hypoglycemia during HD.
Information about the use of antihypertensive and antidiabetic drugs is needed
to prevent the occurrence of hypotension and hypoglycemia in routine
hemodialysis patients.
