HUBUNGAN SELISIH USIA DAN SEX-MATCHING DONOR-RESIPIEN TERHADAP KADAR KREATININ SERUM PADA PASIEN TRANSPLANTASI GINJAL DI RSUP DR. SARDJITO

BACKGROUND
Kidney transplantation is still considered the best treatment option for end stage kidney disease because it improves quality of life and survival of patients when compared with hemodialysis. Various factors can affect the results of kidney transplants, whether in the form of recipient factors, donors, preoperative, intraoperative, or postoperative problems. Other studies show that survival graft among elderly donors is 85.6% at 3 years compared with 90.2% at 3 years on donors who are younger. Women tend to have smaller kidneys with 17?wer nephrons than male kidneys. These facts might be a reference in making an age and sex-matching protocol in selecting candidates for kidney transplant patients.
OBJECTIVES
The objective of this study is to find out whether there is a significant relationship between age difference and sex-matching of donor-recipient serum creatinine levels among post-kidney transplant patients.
MATERIALS AND METHODS
Subjects for this study were 34 patients who have done kidney transplants in Dr. Sardjito Hospital Yogyakarta, Indonesia. Subjects were divided into 8 categories based on the age difference and 4 categories based on sex-matching donor-recipient transplant patients. Serum creatinine level was measured in the 3rd day post-operative. The data then analyzed with one-way ANOVA test.
RESULTS
Among 34 patients, subjects were 24 men and 10 women; 14 patients underwent open nephrectomy and 20 patients underwent laparoscopic. All kidney donors were taken from living donors. Subjects age range was 22-67 years (mean 36.94 years). The donor-recipient relationship consists of 21 (61.76%) unrelated people, 4 (11.76%) child and mother, 3 (8.82%) husband and wife, 5 (14.71%) brother and sister, 1 (2.94%) nephew and uncle. Based on age difference, the lowest creatinine value was in the younger donors with age difference between -5 to -1 year (1.26 mg / dl ± 0.66), whereas the highest was in the older donors with age difference between 11-15 years (4.26 mg / dl ± 0.74) with p-value 0.393. Based on sex matching, the lowest mean creatinine value was in the female donors - female recipients’ group (0.92 mg / dl ± 0.24), while the highest creatinine value was in the male donors - male recipients’ group (3.07 mg / dl ± 1.78) with p-value 0.115.
CONCLUSION
Although the results of this study are not statistically significant, it shows us that younger donor and woman donor-recipient give better outcomes for renal transplantation. The results of this study are certainly widely recognized by confounding factors, such as tiny sample number, patients’ comorbidities, anatomical factors, BMI, HLA mismatch, perioperative complications, duration of ischemic time, etc. Further research needs to be done in the future to obtain more valid results.
