ANALYSIS OF THE COMPLETENESS OF MEDICAL RECORDS WRITTEN BY TREATING DOCTORS FOR WHOLE PERSON IMPAIRMENT (WPI) RATING ASSESSMENT DONE BY THE ASSESSING DOCTOR FOR THIRD-DEGREE INJURIES ON NON- EXTREMITY REGIONS

Latar belakang: AMA Guides telah banyak digunakan di beberapa negara untuk menilai peringkat Whole Person Impairment (WPI) untuk tujuan kompensasi pekerja atau untuk mengklaim asuransi. Cedera tingkat ketiga dapat mengakibatkan cacat sementara atau permanen, baik secara fisik maupun psikologis. Peran dokter yang merawat bertanggung jawab untuk memeriksa secara fisik dan memberikan perawatan atau pengobatan langsung kepada pasien. Oleh karena itu, mereka akan bertugas mengisi rekam medis pasien. Dokter penilai bertanggung jawab untuk melakukan penilaian WPI melalui rekam medis. Tujuan: Untuk mengetahui apakah informasi yang terdapat dalam rekam medis cukup untuk menilai dokter dilakukan penilaian penilaian Whole Person Impairment (WPI). Metode: Jenis penelitian ini adalah deskriptif potong-lintang. Rekam medis pasien cedera derajat tiga pada tahun 2021 akan diperoleh dari RSUP dr. Sarjito. Sampel diambil melalui random sampling. Checklist akan digunakan untuk mengkaji kelengkapan informasi yang dibutuhkan untuk menilai WPI di setiap rekam medis. Hasil: Dari 53 rekam medis yang memenuhi kriteria inklusi dengan rincian 49 rekam medis mengacu pada luka derajat tiga di regio wajah, 0 rekam medis mengacu pada luka derajat tiga di regio tenggorokan, dan 4 rekam medis mengacu pada luka derajat tiga di regio tulang belakang, ditemukan bahwa 37 (69,8%) dari rekam medis tersebut memiliki informasi yang lengkap yaitu yang mencakupi aspek keluhan utama, riwayat, hasil pemeriksaan fisik, dan pemeriksaan penunjang. Dengan adanya informasi yang lengkap maka penilaian WPI dapat dilakukan dengan jumlah skor tertinggi yaitu 21 rekam medis (56,8%) memiliki impairment rating sebanyak 3%. Kesimpulan: Mayoritas rekam medis memiliki informasi yang cukup untuk melakukan penilaian Whole Person Impairment (WPI) yang meliputi aspek keluhan utama, riwayat, pemeriksaan fisik, dan pemeriksaan penunjang. Skor WPI yang rendah mencerminkan kualitas pelayanan rumah sakit yang baik dan dokter yang kompeten.

Background: AMA Guides has been widely used in several countries to assess Whole Person Impairment (WPI) rating for worker's compensation purpose or to claim insurance. A third-degree injury may result in a temporary or permanent disability, both physically and psychologically. The role of treating doctor is accountable for physically examining and providing direct care or treatment to the patient. Hence, they will be in charge of filling in the patient's medical records. The assessing doctor is accountable for doing WPI assessment through the medical records. Objective: To investigate whether the information found in medical records is sufficient for assessing doctor to do Whole Person Impairment (WPI) rating assessment. Methods: This is a descriptive cross-sectional study. Medical records of patients with third-degree injury in 2021 will be obtained from RSUP dr. Sardjito. Samples are taken through random sampling. Checklist will be used to examine the completeness of information needed to assess WPI in each medical record. Results: Out of the 53 medical records that met the inclusion criteria, there are 49 of the medical records referring to third-degree injury in the facial region, 0 medical records referring to third-degree injuries in the throat region, and 4 medical records referring to third-degree injuries in the spine region, it was found that 37 (69,8%) of the medical records have complete information which included aspects of the main complaint, history, results of physical examination, and supporting examinations. With complete information, the WPI assessment can be carried out and the highest score of 21 medical records (56.8%) having an impairment rating of 3%. Conclusion: The majority of medical records have sufficient information to conduct a Whole Person Impairment (WPI) assessment which includes aspects of the chief complaint, history, physical examination, and supporting examinations. A low WPI score reflects the quality of good hospital services and competent healthcare workers.
