Background: Hospital Acquired Infections (HAIs) are serious issues that need prompt and appropriate handling because of their potential to increase mortality and healthcare costs. Implementing HAIs surveillance accompanied by integrated data management is the right step because it provides useful data for stakeholders as a basis for decision-making. However, the current system at Majalaya Hospital does not accommodate integrated HAI data management. District Health Information Software 2 as an integrated data warehouse offers a solution to the disintegration problem of HAIs data management. Objective: To design a prototype of a surveillance information system based on DHIS2 that can meet the needs of HAIs surveillance data management in Majalaya Hospital. Methods: Research and development (R&D) with a prototyping approach in the Rapid Application Development (RAD) software development method. The study was conducted at Majalaya Hospital, Bandung Regency, from March-September 2022. The subjects of this study were Infection Prevention and Control Nursing (IPCN) and Infection Prevention and Control Link Nursing (IPCLN) officers. The object of this research is the HAIs surveillance information system based on DHIS2 Tracker. Results: HAIs surveillance information system prototypes have been designed and developed through the process of system requirements analysis, process design, metadata design, testing, and evaluation. Evaluation of menu and user interface testing shows that the prototype system functionality is running well and users express overall satisfaction with the menu and user interface of the system prototype. Conclusion: System prototypes have been developed according to user requirements. However, testing and evaluation are limited to menus and user interfaces, so further exploration and development are required.
