KORELASI ANTARA ERICA SCORE DENGAN VOLUME CORTEX ENTORHINAL PADA MRI VOLUMETRIK CEREBRAL PASIEN DEMENSIA

Latar Belakang: Demensia adalah diagnosis klinis atas dasar penurunan kognitif progresif. Penyakit Alzheimer adalah bentuk paling umum dari demensia. Pengukuran atrofi di cortex entorhinal dan subregional hippocampus cenderung menjadi penanda biologis awal fase neurodegeneratif penyakit Alzheimer. Beberapa teknik dapat dilakukan untuk mengukur atrofi otak menggunakan MRI dan CT. Volumetri manual dianggap sebagai standar emas tetapi lebih menghabiskan waktu. Volumetri otomatis cenderung lebih cepat dan dapat diterapkan secara luas, tetapi mungkin rentan terhadap variabilitas pemindai dan protokol pemindaian. Tujuan: Mengetahui korelasi antara ERICA score dengan volume cortex entorhinal pada MRI volumetrik cerebral pasien demensia. Bahan dan Metode: Penelitian ini adalah penelitian observasional analitik korelasi yang menggunakan data sekunder. Subyek penelitian adalah pasien yang terdiagnosis demensia oleh klinisi dan telah menjalani pemeriksaan MRI kepala di Instalasi Radiologi RSUP Dr. Sardjito Yogyakarta dari Januari 2021 hingga Juni 2022. Subyek penelitian diambil dengan menggunakan metode consecutive non random sampling. Hasil: Dari 32 subyek penelitian, rata-rata usia subyek adalah 67,28 tahun. Terdapat 6 orang (18,7%) dengan tipe demensia Alzheimer, 11 orang (34,4%) dengan tipe demensia vaskuler, 7 orang (21,9%) dengan tipe demensia mixed type dan 8 orang (25%) dengan tipe demensia unspecified. Dari uji korelasi Spearman, didapatkan nilai koefisien korelasi (r) = -0,434 dengan nilai p = 0,013 (p<0,05). Kesimpulan: Terdapat korelasi yang signifikan secara statistik antara ERICA score dengan volume cortex entorhinal pada MRI volumetrik cerebral pasien demensia. Mayoritas tipe demensia subyek penelitian adalah demensia vaskuler dengan sebagian besar subyek berjenis kelamin laki-laki dengan usia >65 tahun. Background: Dementia is a clinical diagnosis on the basis of progressive cognitive decline. Alzheimer's disease is the most common form of dementia. Measurement of atrophy in the entorhinal cortex and subregional hippocampus is likely to be a biological marker of the early neurodegenerative phase of Alzheimer's disease. Several techniques can be used to measure brain atrophy using MRI and CT. Manual volumetry is considered the gold standard but is more time consuming. Automated volumetric tends to be faster and widely applicable, but may be susceptible to scanner and scan protocol variability. Objectives: To understand the correlation between ERICA score and entorhinal cortex volume on cerebral volumetric MRI of dementia patients. Materials and Methods: This research is a correlation analytic observational study that uses secondary data. The research subjects are patients diagnosed with dementia by clinicians and had undergone a head MRI examination at the Radiology Installation of Dr. Sardjito Hospital in Yogyakarta from January 2021 to June 2022. Research subjects were taken using consecutive non-random sampling method. Results: Of the 32 research subjects, the average age of the subject is 67.28 years. There are 6 people (18.7%) with Alzheimer's dementia, 11 people (34.4%) with vascular dementia, 7 people (21.9%) with mixed type dementia and 8 people (25%) with unspecified dementia. From the Spearman correlation test, the correlation coefficient value (r) = -0.434 with p value = 0.013 (p<0.05). Conclusion: There is a statistically significant correlation between ERICA score and entorhinal cortex volume on cerebral volumetric MRI of dementia patients. The majority of the dementia types of the research subjects are vascular dementia with most of the subjects being male with age >65 years.
