Perhitungan Unit Cost dengan Pendekatan Activity Based Costing pada Layanan Autologous Hematopoietic Stem Cell Transplantation dengan Diagnosa Multiple Myeloma Berbasis Clinical Pathway di RSUP Dr. Kariadi Semarang

RSUP Dr. Kariadi Semarang saat ini tengah mengembangkan layanan unggulan baru yaitu Cangkok Sumsum Tulang (CST). Layanan CST termasuk dalam layanan yang high risk, high cost dan problem prone. Jumlah pasien yang dilakukan CST sejak tahun 2012 hingga juli 2021 tercatat sebanyak 27 orang, 22 diantaranya dilakukan Autologous Stem Cell Transplantation. Berbagai upaya untuk peningkatan mutu dan pengembangan layanan telah dilakukan, namun sampai saat ini layanan CST belum memiliki Clinical Pathway. Data keuangan menunjukkan perbedaan jumlah nominal pada billing akhir pasien, hal ini dimungkinkan adanya variasi dalam pemberian layanan pada kasus dan tindakan yang sama dalam layanan CST. Oleh karenanya pembuatan Clinical pathway pada layanan CST diperlukan dalam rangka kendali mutu dan kendali biaya. Clinical pathway menyajikan segala aktivitas sejak pasien masuk sampai dengan pulang, maka metode Activity Based Costing (ABC) digunakan sebagai perhitungan unit cost pada layanan ini. Tujuan penelitian ini adalah untuk membuat konsep clinical pathway dan menghitung unit cost layanan Autologous Stem Cell Transplantation pada episode conditioning dan infusion stem cell dengan diagnosa Multiple Myeloma (MM) berbasis Clinical Pathway di RSUP Dr. Kariadi Semarang. Penelitian mix method dengan rancangan desain sekuensial eksploratori. Hasil penelitian menunjukkan bahwa Clinical pathway memuat segala aktivitas profesional pemberi asuhan sejak pasien datang sampai dengan pasien diijinkan pulang. Aktivitas mulai dari asesmen awal sampai dengan perencanaan pulang merupakan gambaran dari aktivitas pemberi asuhan sesuai kewenangan masing-masing dengan pasien sebagai centre care. Perhitungan unit cost dengan metode ABC berdasarkan clinical pathway diharapkan mampu menggambarkan biaya yang sesungguhnya dalam proses perawatan pasien. Aktivitas yang berhubungan langsung dengan pelayanan yaitu pemberian terapi intravena menempati proporsi tertinggi dalam perhitungan unit cost ini, sementara biaya overhead hanya sebesar 3.6% dari keseluruhan total unit cost yang dihitung. Total unit cost pada penelitian ini sebesar Rp 141,422,703. Sementara perhitungan unit cost dengan menggunakan terapi yang sesuai restriksi BPJS sejumlah Rp 82,134,006. Kesimpulan dari penelitian bahwa aktivitas yang dilakukan oleh profesional pemberi asuhan dalam clinical pathway sudah sesuai dengan standar yang diharapkan. Perhitungan unit cost sesuai clinical pathway lebih tinggi dari tarif INA CBG's yang berlaku, meskipun nilai pada perhitungan unit cost yang menggunakan terapi sesuai restriksi BPJS lebih rendah dari tarif INA CBG's, namun belum bisa dibandingkan karena belum merupakan tarif final layanan.

RSUP Dr. Kariadi Semarang is currently developing a new flagship service, namely Bone Marrow Transplant (BMT). BMT services are included in high risk, high cost and problem prone services. The number of patients who underwent BMT from 2012 to July 2021 was 27 people, 22 of whom underwent Autologous Stem Cell Transplantation. Various efforts have been made to improve the quality and development of services, but until now this BMT service does not yet have a Clinical Pathway. From the hospital's financial data, it shows the difference in nominal amounts in final billing of patients, it is possible that there are variations in service delivery in the same case and action in BMT services. Therefore, making Clinical pathways for BMT services is needed in context of quality control and cost control. The clinical pathway presents all activities starting from patient arrival until discharge, so the Activity Based Costing (ABC) method is used as a unit cost calculation. The purpose of this study was to create a clinical pathway concept and calculate the unit cost of the Autologous Stem Cell Transplantation service for episode conditioning and stem cell infusion with the diagnosis of Multiple Myeloma (MM) based on Clinical Pathway at RSUP Dr. Kariadi Semarang. The research Method used Mix method research with exploratory sequential design. Research result shows that The Clinical pathway contains all the professional activities of care giver from the time the patient arrives until the patient is allowed to go home. Activities starting from initial assessment to discharge planning are a description of the activities of the care giver according to their respective authorities with the patient as the centre of care. Calculation of unit costs using the ABC method based on clinical pathways is expected to be able to describe the actual costs in the patient care process. Activities that are directly related to services, namely the administration of intravenous therapy, occupy the highest proportion in the calculation of this unit cost, while overhead costs are 3.6% of the total calculated unit cost. The total unit cost in this study is IDR 141,422,703. Meanwhile, the unit cost calculation using therapy according to BPJS restrictions is IDR 82,134,006. The conclusion of this research are activities carried out by professional care providers in clinical pathways are in accordance with the expected standards. The unit cost calculation according to the clinical pathway is higher than the applicable INA CBG's rate, although the value in the unit cost calculation using therapy according to BPJS restrictions is lower than the INA CBG's rate, it cannot be compared because it is not the final service tariff.
