The management of the COVID-19 vaccine through the cold chain system plays an essential role in maintaining the quality of the vaccine and ensuring its availability in the community. Data collection was obtained from qualitative data by conducting observations and interview processes, as well as documents obtained from data on the use of COVID19 vaccines throughout 2020-2021 and the use of vaccine stelling cards. Qualitative data was processed using the triangulation method. The results obtained indicate that the suitability of the COVID-19 vaccine cold chain management system by the UPT Pharmacy and Medical Devices of the Yogyakarta City Health Office with guidelines is 92.3%. The evaluation of the quality of the management of the COVID-19 vaccine showed good results with the percentage of storage temperature compliance is 100%, the average percentage of vaccine void time is 0%, the percentage of damaged vaccines is 0%, and the percentage of total expired COVID-19 vaccines is 0.0505%. These results indicate that the management of the COVID-19 vaccine at the UPT Pharmacy and Medical Devices of the Yogyakarta City Health Office has been going well and has followed the required guidelines.
