Cost of Illness pada Pasien Diabetes Melitus Tipe 2: Systematic Review

Prevalensi kasus diabets melitus terus meningkat beberapa dekade terakhir. Terapi penyakit diabetes melitus tipe 2 berlangsung terus-menerus seumur hidup sehingga membutuhkan biaya yang sangat besar. Tujuan penelitian ini adalah untuk mengetahui total biaya yang diperlukan untuk pengobatan diabetes melitus tipe 2 serta mengetahui pengaruh komplikasi diabetes melitus terhadap biaya pengobatan. Metode yang digunakan dalam penelitian ini adalah studi systematic review, dengan tiga database yaitu Pubmed, Scopus, dan Google scholar. Sebanyak 1323 artikel diambil dan ditinjau. Enam belas artikel dipilih karena memenuhi kriteria. Metode analisis secara deskriptif dengan menggabungkan seluruh data yang diperoleh dan dibahas secara keseluruhan untuk mendapatkan gambaran terkait jumlah biaya yang dibutuhkan dari yang terendah hingga tertinggi pada penyakit diabetes melitus tipe 2. Biaya untuk pengobatan DM tipe 2 per pasien dalam satu tahun bervariasidari biaya langsung dibagi menjadi biaya medis langsung yang diperlukan antara USD 74,11 sampai dengan USD 6240,30. Biaya non medis langsung juga bervariasi antara USD 30,33 sampai dengan USD 1521,59. Biaya tidak langsung berkisar antara USD 7,98 sampai dengan USD 6432,38. Adanya komplikasi diabetes melitus tipe 2 berpengaruh meningkatkan biaya pada pengobatan pasien diabetes melitus tipe 2. Komplikasi mikrovaskular meningkat antara 0,5 kali sampai dengan 2,3 kali. Makrovaskular meningkat antara 0,3 kali sampai dengan 2,7 kali, dan keduanya meningkat antara 0,6 kali sampai dengan 9,7 kali.

The prevalence of diabetes mellitus has continued to increase in the last few decades. Type 2 diabetes mellitus treatment that lasts a lifetime requires a high cost. This research aimed to total cost required for type 2 diabetes mellitus treatment and determine the effect of complications of diabetes mellitus on the cost of treatment. The research used a systematic review with three databases: Pubmed, Scopus, and Google scholar. A total of 1323 articles were retrieved and reviewed. Sixteen journal articles were selected because they met the criteria. Furthermore, the descriptive analysis method employed by combining and discussing all the data obtained related to the amount of cost required from the lowest to the highest in type 2 diabetes mellitus. The cost for type 2 DM treatment per patient in one year varies from direct costs divided into direct medical costs for about USD 74.11 to USD 6240.30. Direct non-medical costs also vary between USD 30.33 to USD 1521.59. Indirect costs ranged from USD 7.98 to USD 6432.38. Complications of type 2 diabetes mellitus affect the cost increase of treating patients with type 2 diabetes mellitus. Microvascular complications increased between 0.5 to 2.3 times, macrovascular complications increased between 0.3 to 2.7 times, and both increased between 0.6 to 9.7 times.
