Pengaruh Klorheksidin sebagai Bahan Obat Kumur terhadap Iritasi Jaringan Periodontal

Obat kumur klorheksidin dikenal sebagai gold standar serta dianjurkan secara teratur sebagai produk kontrol plak kimia, namun diketahui memiliki efek samping. Efek samping klorheksidin terhadap jaringan sekitar tergantung pada dosis, frekuensi, serta durasi penggunaan. Jaringan periodontal merupakan salah satu jaringan yang penting dalam rongga mulut. Beberapa penelitian membahas mengenai toksisitas klorheksidin terhadap beberapa sel jaringan periodontal serta efek samping dari klorheksidin terhadap jaringan periodontal. Narrative review ini bertujuan untuk mengkaji pengaruh klorheksidin dalam obat kumur terhadap respon seluler jaringan periodontal serta mengetahui efek samping obat kumur klorheksidin dalam potensinya menyebabkan iritasi jaringan periodontal. Pubmed, Science Direct, dan Google Scholar merupakan database yang digunakan untuk mencari literatur yang diterbitkan pada mulai tahun 2016 hingga 2022. Literatur yang digunakan merupakan artikel penelitian, review, case report, atau clinical trial, dalam bahasa Inggris maupun bahasa Indonesia. Pencarian dilakukan dengan memasukkan kata kunci chlorhexidine, mouthwash, inflammation,irritation, toxic, response, adverse effect, side effect, periodontal, klorheksidin, obat kumur, inflamasi, iritasi, toksik, respon, dan efek samping, yang kemudian dipadukan dengan menggunakan operator Boolean (AND dan OR). Total referensi yang digunakan yaitu sebanyak 53. Pada penggunaan dosis tinggi dan durasi lama, klorheksidin dilaporkan bersifat sitotoksik pada sel jaringan periodontal serta dapat menimbulkan reaksi efek negatif. Berdasarkan review yang telah dilakukan, kesimpulan yang didapatkan adalah klorheksidin dalam obat kumur dilaporkan memberikan respon seluler berupa toksisitas terhadap sel pada jaringan periodontal bergantung pada konsentrasi dan waktu paparan serta obat kumur klorheksidin diketahui memiliki efek samping yang dapat berpotensi menyebabkan iritasi pada jaringan periodontal.

Chlorhexidine as the mouthwash gold standard is recommended regularly for chemical plaque control product, but is known to have side effect. The side effect chlorhexidine mouthwash on surrounding tissue depends on the dose, frequency, and duration of use. Periodontal tissue is one of the important tissues in the oral cavity. Several studies discuss the toxicity of chlorhexidine to some periodontal tissue cells and the side effects of chlorhexidine on periodontal tissue. This narrative review aims to examine the effect of chlorhexidine in mouthwashes on the cellular response of periodontal tissue and determine the side effects of chlorhexidine mouthwash in its potential to cause periodontal tissue irritation. This narrative review articles sourced from Pubmed, Science Direct, and Google Scholar published from 2016 to 2022. It used research articles, reviews, case reports, or clinical trials, in English and Indonesian. The search was carried out by entering the keywords, including chlorhexidine, mouthwash, inflammation, irritation, toxic, response, adverse effect, side effect, periodontal, klorheksidin, obat kumur, inflamasi, iritasi, toksik, respon, and efek samping by using Boleean Operator (AND and OR). The 53 references were used. The use of the chlorhexidine at high doses and long duration, has been reported to be cytotoxic to periodontal tissue cells and can cause negative reactions. The conclusion of this narrative review was chlorhexidine in mouthwash are reported to provide a cellular response in the form of toxicity to cells in the periodontal tissue depending on the concentration and time exposure. Chlorhexidine as the mouthwash is known to have side effects that can potentially cause irritation to the periodontal tissue.
