The Difference Between Serum Endothelin-1 Levels In Patients With St-Elevation Myocardial Infarction And Stable Coronary Artery Disease

Latar Belakang: Endotelin-1 merupakan peptida terlarut yang memiliki kemampuan vasokonstriksi poten yang mempengaruhi baik arteri koroner maupun arteri perifer. Peningkatan endotelin-1 telah diamati dengan adanya plak aterosklerotik dan infark miokard. Kajian kadar endotelin-1 pada PJK stabil masih sangat terbatas, sebagian besar penelitian difokuskan untuk mengeksplorasi hubungan kadar endotelin-1 pada Infark Miokard. Oleh karena itu, perlu dilakukan penelitian yang membandingkan kadar endotelin-1 pada CAD stabil dengan kadar endotelin-1 pada STEMI. Tujuan: Untuk mengetahui ada tidaknya hubungan antara kadar endotelin-1 pada pasien CAD stabil dibandingkan dengan pasien STEMI. Metode: Studi potong lintang dilakukan dengan data sekunder dari register Hartopo et al. (2016) tesis. Sampel plasma diambil dari vena antecubital selama rawat inap di RSUP Dr. Sardjito Yogyakarta. Sampel plasma diukur dengan metode ELISA menggunakan IBL ET-1. Data dianalisis berdistribusi normal dengan uji Kolmogorov-Smirnov. Subyek dibagi menjadi dua kelompok, berdasarkan diagnosis STEMI atau CAD Stabil. Uji-t dan uji Mann-Whitney akan dilakukan sesuai dengan pola distribusi data untuk membandingkan mean antara kedua kelompok perbedaannya signifikan jika p-value < 0,05. Hasil: Total subjek adalah 308; CAD stabil berjumlah 183 dan STEMI berjumlah 125 subjek. Nilai median subjek dengan STEMI adalah 2,930 pg/mL dan CAD stabil adalah 2,838 pg/mL. Analisis statistik menemukan tidak ada perbedaan kadar endotelin-1 antara STEMI dan CAD (p-value=0,372). Kesimpulan: Tidak ada perbedaan bermakna antara endotelin-1 pada subjek dengan CAD stabil dan STEMI

Background: Endothelin-1 is a soluble peptide that has potent vasoconstriction capability affecting both the coronary artery and peripheral artery. The increment of endothelin-1 has been observed in the presence of atherosclerotic plaque and myocardial infarct. The study of endothelin-1 level in stable CAD is quite limited, most of the study focused on exploring the relationship of endothelin-1 level in Myocardial Infarction. Hence, there is a need to make a study that compares the level of endothelin-1 in stable CAD with the level of endothelin-1 in STEMI. Objective: To investigate whether there is any correlation between the level of endothelin-1 in patients with stable CAD compared to patients with STEMI. Method: a cross-sectional study was done with the secondary data from the registry of Hartopo et al. (2016) thesis. Plasma samples were taken from the antecubital veins during hospitalization in RSUP Dr. Sardjito, Yogyakarta. The plasma sample was measured by the ELISA method using IBL ET-1. The data were analyzed for normal distribution with the Kolmogorov-Smirnov test. Subjects were divided into two groups, based on the diagnosis of STEMI or Stable CAD. T-test and Mann-Whitney test will be done in accordance with data distribution pattern to compare the mean between the two groups the difference is significant if p-value <0.05. Results: The total subject is 308; stable CAD amounted to 183 and STEMI amounted to125 subjects. The median value of subjects with STEMI is 2.930 pg/mL and stable CAD is 2.838 pg/mL. Statistical analysis found there is no difference in endothelin-1 level between STEMI and CAD (p-value=0.372). Conclusion: There is no significant difference between endothelin-1 in subjects with stable CAD and STEMI
