Peran Kandungan Vasculae Endothelial Growth Factor Pada Platelet-Rich Fibrin terhadap Penyembuhan Jaringan Periodontal

Penyakit periodontal merupakan interaksi kompleks inflamasi pada jaringan periodontal. Terapi periodontal tidak dapat mengembalikan struktur jaringan periodontal secara sempurna sehingga dikembangkan terapi regeneratif dengan tambahan Platelet-Rich Fibrin (PRF) yang merupakan konsentrat platelet generasi kedua yang dapat melepas berbagai macam growth factor, salah satunya adalah Vascular Endothelial Growth Factor (VEGF). Narrative review ini bertujuan untuk mengetahui peran VEGF pada PRF terhadap penyembuhan jaringan periodontal. Penelusuran literatur pada narrative review ini menggunakan database Pubmed, Google Scholar, Science Direct, dan WILEY Online Library. Kriteria inklusi dalam pencarian literatur adalah jurnal review, jurnal case report, dan textbook ilmiah berbahasa Indonesia atau Inggris yang diterbitkan dalam rentang tahun 2011-2021, sedangkan kriteria eksklusi adalah sumber pustaka tidak dapat diakses, tidak tersedia dalam satu teks utuh, dan tidak menjelaskan metode penelitian. Total literatur yang di-review untuk membahas peran VEGF dalam PRF terhadap penyembuhan jaringan periodontal adalah 16 literatur. Hasil dan kesimpulan review menunjukkan bahwa VEGF berperan dalam regenerasi jaringan periodontal yaitu meningkatkan angiogenesis pada gingiva, meregulasi osteoblas pada tulang alveolar, mendukung proliferasi sementoblas pada sementum, dan mempromosikan terbentuknya perlekatan b aru pada ligamen periodontal. Pelepasan Vascular Endothelial Growth Factor (VEGF) pada jenis- jenis Platelet Rich Fibrin (PRF) yang dibahas dalam narrative review ini ditemukan paling tinggi terdapat pada A-PRF+ diikuti oleh L-PRF, A-PRF, PRFM, PRF, PRP, CGF, L-PRP, dan i-PRF.

Periodontal disease was a complex interaction of inflammation in the periodontal tissues. Periodontal therapy could not completely restore the structure of the periodontal tissue, so regenerative therapy was developed with the addition of Platelet-Rich Fibrin (PRF) which was a second-generation platelet concentrate that could release various growth factors, one of which was the Vascular Endothelial Growth Factor (VEGF). This narrative review aimed to determine the role of VEGF in PRF in periodontal tissue healing. The literature search in this narrative review used the Pubmed, Google Scholar, Science Direct, and WILEY Online Library databases. The inclusion criteria in the literature search were review journals, case report journals, and Indonesian or English scientific textbooks published in 2011-2021, while the exclusion criteria were inaccessible library sources, not available in one complete text, and did not clearly explain the research method used. The total literature reviewed to discuss the role of VEGF in PRF in periodontal tissue healing were 16 works of literature. The results and conclusion of the review showed that VEGF played an important role in the regeneration of periodontal tissue which includes increasing the number of angiogenesis in gingival tissues, regulating osteoblast activity in alveolar bone, supporting cementoblast proliferation in cementum, and encouraging the formation of new attachment to the periodontal ligament. The release of VEGF Vascular Endothelial Growth Factor (VEGF) in the types of Platelet Rich Fibrin (PRF) discussed in this narrative review was found highest in A-PRF+ followed by L-PRF, A-PRF, PRFM, PRF, PRP, GCF, L-PRP, and i-PRF.
