Pengaruh Jenis Konsentrat Platelet terhadap Pola Pelepasan Transforming Growth Factor Beta 1 (TGF-beta1) pada Penyembuhan Penyakit Periodontal

Konsentrat platelet merupakan salah satu material yang penggunaannya luas dalam prosedur penyembuhan jaringan periodontal. Seiring waktu, konsentrat platelet terus mengalami modifikasi dalam prosedur sentrifugasi untuk meningkatkan potensi regenerasinya dengan memanfaatkan kandungan growth factors yang tinggi. Growth factor ketiga yang dilepas terbesar oleh konsentrat platelet adalah transforming growth factor beta 1 (TGF-beta1) yang memiliki peranan penting pada tahap awal diferensiasi osteoblas untuk maturasi matriks ekstraseluler selama proses regenerasi jaringan periodontal. Tujuan dari narrative review ini adalah untuk mengkaji pengaruh jenis konsentrat platelet terhadap pola pelepasan TGF-beta1 pada penyembuhan penyakit periodontal. Artikel yang diulas dalam narrative review ini diperoleh dari 5 database yang berbeda, antara lain PubMed, ScienceDirect, Google Scholar, SAGE Journals, dan WILEY Online Library. Kata kunci yang digunakan adalah kombinasi dari platelet concentrate, platelet-rich plasma (PRP), platelet-rich fibrin (PRF), transforming growth factor beta 1 (TGF-beta1), dan growth factor release. Artikel diseleksi berdasar kriteria inklusi dan eksklusi yang telah ditentukan, terdapat 22 artikel in vitro yang dimasukkan dalam narrative review ini. Hasil narrative review ini menunjukkan setiap jenis konsentrat platelet memiliki pola pelepasan TGF-beta1 yang berbeda. Perbedaan pola pelepasan tersebut dipengaruhi oleh modifikasi prosedur preparasi seperti kebutuhan terhadap penggunaan aktivator dan jenisnya, perubahan kecepatan dan waktu sentrifugasi, jenis tabung, dan angulasi tabung sehingga merubah durasi dan level pelepasan TGF-beta1 yang mempengaruhi potensinya selama tahap awal diferensiasi osteoblas yaitu maturasi matriks ekstraseluler untuk osteogenesis pada regenerasi jaringan periodontal. Kesimpulan narrative review ini adalah jenis konsentrat platelet mempengaruhi pola pelepasan TGF-beta1 pada penyembuhan penyakit periodontal.

Platelet concentrate is one of the most widely utilized materials in procedures of periodontal tissue healing. Over the time, platelet concentrates continue to undergo modifications in its centrifugation procedure to increase their regeneration potential by taking advantage of the high content of growth factors that present. The third largest growth factor released by platelet concentrate is transforming growth factor beta 1 (TGF-beta1) which has an important role in early stages of osteoblast differentiation for extracellular matrix maturation during periodontal tissue regeneration process. The aim of this narrative review was to elaborate the effect of various types of platelet concentrate on TGF-beta1 release pattern in periodontal disease treatment. The articles that reviewed in this narrative review were obtained from 5 different databases, such as PubMed, ScienceDirect, Google Scholar, SAGE Journals, and WILEY Online Library. The used key words were the combination of platelet concentrate, platelet-rich plasma (PRP), platelet-rich fibrin (PRF), transforming growth factor beta 1 (TGF-beta1), and growth factor release. Articles were selected using the predetermined inclusion and exclusion criteria, a total of 22 in vitro articles were included in this narrative review. The result of this narrative review showed that various types of platelet concentrate have different TGF-beta1 release pattern. The difference on the release pattern influenced by modifications in preparation procedure such as the form of the need for the use of activators and their types, changes to centrifugation speed and time, types of the tube, and tube angulation, thereby changing the duration and level of TGF-beta1 released which affected its potential to support osteogenesis in periodontal tissue regeneration. The conclusion of this narrative review was that various types of platelet concentrate affected TGF-beta1 release pattern in periodontal disease treatment.
