PENGARUH PREPARASI SENTRIFUGASI HORIZONTAL DENGAN BERBAGAI MODIFIKASI TERHADAP KARAKTERISTIK PLATELET-RICH FIBRIN

Prosedur regeneratif diterapkan pada proses pemulihan struktur jaringan yang hilang pada penyakit jaringan periodontal. Komponen darah sebagai biomaterial autologous berpotensi mendukung terjadinya aktivitas biologis regenerasi jaringan. Platelet-rich fibrin merupakan konsentrat trombosit berupa matriks fibrin yang diperkaya sel trombosit, leukosit, serta melepaskan faktor pertumbuhan. Untuk meningkatan kemampuan regeneratifnya, dilakukan berbagai modifikasi protokol preparasi, termasuk angulasi sentrifugasi horizontal. Tujuan penyusunan narrative review ini yaitu untuk menelaah secara ilmiah pengaruh teknik preparasi sentrifugasi horizontal dengan berbagai modifikasi terhadap karakteristik PRF. Artikel yang ditinjau diambil dari beberapa database Cochrane, Nature, Pubmed, ScienceDirect, SpringerLink, dan Wiley Online Library. Pencarian artikel menggunakan kata kunci yang telah ditentukan menggunakan operator Boolean (AND, OR, dan NOT). Kemudian artikel dipilih berdasarkan kriteria inklusi serta kriteria eksklusi. Penulis melakukan pemindaian untuk menghindari adanya duplikasi artikel. Terdapat enam parameter karakteristik PRF yang diamati. Aplikasi teknik sentifugasi horizontal dengan berbagai modifikasi mampu menghasilkan PRF dengan sifat biokompatibilitas yang baik, distribusi sel trombosit dan leukosit yang meluas dan homogen, potensi jumlah sel trombosit dan leukosit yang lebih tinggi, mampu melepaskan faktor pertumbuhan yang banyak dan berkepanjangan, memiliki struktur yang elastis, resisten, dan mudah dibentuk, serta menunjukkan aktivitas biologis yang positif. Meskipun demikian, diperlukan studi lebih lanjut untuk mengonfirmasi manfaat sentrifugasi horizontal terhadap regenerasi jaringan secara in vivo maupun klinis.

Regenerative procedures are applied to restore damaged tissue in periodontal tissue disease. Blood components as autologous biomaterials may support tissue regeneration activity. Platelet-rich fibrin is a platelet concentrate in the form of a fibrin matrix enriched by platelet, leukocytes, and releases growth factors. To improve its regenerative ability, various modifications of the preparation protocol were carried out, including horizontal centrifugation angulation. This narrative review aimed to examine scientifically the effect of the horizontal centrifugation preparation technique with various modifications on the characteristics of the PRF. Reviewed articles were taken from Cochrane, Nature, Pubmed, ScienceDirect, SpringerLink, and Wiley Online Library databases. Predetermined keywords were used for article searching with Boolean operators (AND, OR, and NOT). Then articles were selected based on inclusion criteria and exclusion criteria. Scanning by the author was done to avoid duplicate articles. There are six parameters of PRF characteristics to be observed. The application of horizontal centrifugation technique with various modifications was able to produce PRF with good biocompatibility properties, wide and homogeneous distribution of platelets and leukocytes, potential for higher platelets and leukocytes cell count, abundant and prolonged release of growth factors; elastic, resistant, and easy to form structure; and positive biological activity. However, further studies are needed to ascertain the benefits of horizontal centrifugation on tissue regeneration in vivo and clinically.
