KORELASI KADAR ADIPONEKTIN DAN HOMA-IR PADA IBU HAMIL DENGAN DIABETES MELLITUS GESTASIONAL

Latar belakang: Diabetes melitus gestasional (DMG) merupakan komplikasi kehamilan yang paling umum terjadi. Prevalensi DMG di dunia berkisar antara 1-36%. Diabetes mellitus gestasional meningkatkan mortalitas dan morbiditas baik bagi ibu maupun bayi. Resistensi insulin yang merupakan salah satu patofisiologi utama terjadinya DMG. Indeks HOMA-IR merupakan perhitungan matematis yang dapat menggambarkan keadaan resisitensi insulin dalam tubuh. Resistensi insulin diperberat oleh beberapa faktor, salah satunya adalah kadar adiponektin. Terdapat beberapa penelitian dengan hasil yang beragam mengenai korelasi antara adiponektin dan HOMA-IR. Tujuan : Untuk mengetahui adakah korelasi antara adiponektin dengan HOMA-IR pada ibu hamil dengan DMG. Metode: Penelitian ini merupakan studi observasional analitik dengan desain potong lintang. Penelitian ini melibatkan wanita hamil usia gestasi 24-28 minggu dengan DMG berdasarkan kriteria IADPSG dan diukur indeks HOMA-IR serta adiponektin. Hasil: Penelitian ini melibatkan 25 subjek ibu hamil yang memenuhi kriteria inklusi dan ekslusi. Rerata usia ibu hamil adalah 33 tahun dengan Sebagian besar adalah kelompok usia < 35 tahun, mayoritas adalah multigravida, tidak memiliki riwayat abortus dan riwayat keluarga DM. berdasarkan IMT, mayoritas ibu hamil memiliki IMT diatas normal. Tidak terdapat korelasi antara adiponektin dengan indeks HOMA-IR pada ibu hamil DMG (r=0,313; p=0,127). Simpulan: Tidak terdapat korelasi antara adiponektin dengan HOMA-IR pada ibu hamil dengan DMG.

Background: Gestational diabetes mellitus (GDM) is the most common complication of pregnancy. The prevalence of DMG in the world ranges from 1-36%. Gestational diabetes mellitus increases mortality and morbidity for both mother and baby. Insulin resistance is one of the main pathophysiologies of GDM. The HOMA-IR index is a mathematical calculation that can describe the state of insulin resistance in the body. Insulin resistance is exacerbated by several factors, one of which is adiponectin levels. There are several studies with mixed results regarding the correlation between adiponectin and HOMA-IR. Objective: To determine whether there is a correlation between adiponectin and HOMA-IR in pregnant women with DMG. Methods: This is an analytical observational study with a cross-sectional design. This study involved pregnant women aged 24-28 weeks with GDM based on the IADPSG criteria. 25 pregnant women met inclusion and exclusion criteria. They were investigated for baseline characteristic such as age, gravida, history of abortus, family history of DM and BMI at the moment of diagnosis. All subjects were examined for fasting glucose, 1- and 2-hour glucose after TTGO, fasting insulin, adiponectin and calculated for HOMA-IR index. Results: The mean age of pregnant women is 33 years with most of the age group < 35 years, the majority are multigravida, have no history of abortion and family history of DM. based on BMI, the majority of pregnant women have a normal BMI. There was no correlation between adiponectin and the HOMA-IR index in GDM pregnant women (r=0,313; p=0,127). Conclusion: There is no correlation between adiponectin and HOMA-IR in pregnant women with GDM.
