Peran Prognosis Skor COP-NLR (Combination of Platelet Count and Neutrophil to Lymphocyte Ratio) Pada Penderita Kanker Nasofaring Stadium III-IVB

Latar belakang : Penanda prognosis untuk karsinoma nasofaring (KNF) masih sangat dibutuhkan namun penanda yang ada saat ini yaitu DNA virus Epstein-Barr butuh biaya tinggi dan persyaratan teknis membatasi aplikasi pada pelayanan pasien. Sel-sel inflamasi berperan penting dalam perkembangan suatu kanker dan hal ini telah ditunjukkan dalam berbagai tumor padat bahwa sel-sel inflamasi mempengaruhi prognosis pasien. Skor COP-NLR (kombinasi jumlah trombosit dan perbandingan jumlah neutrofil dan limfosit) darah tepi mudah didapat dan juga bisa mencerminkan respon inflamasi inang. Tujuan : Menilai skor COP-NLR darah tepi sebelum terapi sebagai penanda prognosis pada pasien KNF. Metode : Penelitian kohort retrospektif dengan menggunakan data dari registri klinis pasien KNF yang terdiagnosis sejak tahun 2007-2016 di RSUP Dr. Sardjito Yogyakarta. Analisis kesintasan dengan kurva Kaplan-Meier regresi Cox digunakan untuk menguji pengaruh faktor pasien (usia, jenis kelamin dan status gizi), faktor tumor (ukuran tumor, lokasi dari limfonodi leher yang terpengaruh), faktor terapi (jenis terapi), faktor klinis (anemia), skor COP-NLR terhadap kesintasan pasien KNF. Hasil : Sebanyak 358 pasien KNF stadium III-IVB dianalisis dalam penelitian ini. Penentuan skor COP-NLR menggunakan batas NLR 4,28 dan jumlah tombosit 496 x 10 pangkat 3 /uL. Jika NLR kurang dari atau sama dengan 4,28 dan jumlah trombosit kurang dari atau sama dengan 496 x 10 pangkat 3 /uL maka skornya 0. Jika NLR > 4,28 atau jumlah trombosit > 496 x 10 pangkat 3 /uL maka skornya 1. Jika NLR > 4,28 dan jumlah trombosit > 496 x 10 pangkat 3 /uL maka skornya 2. Kesintasan hidup 2 tahun pada pasien KNF stadium III-IVB dengan skor 0, 1, 2 adalah 72,2%, 57,3%, dan 41,4% (p = 0,002). Skor COP-NLR yang lebih tinggi (skor 1-2 vs skor 0) menunjukkan penurunan kesintasan hidup 2 tahun dengan HR 1,887 (95%IK 1,267-2,81 dan p = 0,002). Simpulan : Pasien KNF stadium III-IVB dengan skor COP-NLR 0 sebelum terapi memiliki kesintasan hidup 2 tahun yang lebih baik dibanding pasien dengan skor COP-NLR 1 dan 2.

Background : Prognostic markers for nasopharyngeal carcinoma (NPC) are still urgently needed but current marker of Epstein-Barr virus DNA require high costs and technical requirements limit applications in patient care. Inflammatory cells play an important role in the development of a cancer and it has been shown in various solid tumors that inflammatory cells influence the patient's prognosis. COP-NLR score peripheral blood are easily obtained and can also reflect host inflammatory responses. Objective : To assess peripheral blood COP-NLR score before therapy as a prognostic marker in NPC patients. Methods : A retrospective cohort study using data from the clinical registry of NPC patients diagnosed from 2007-2016 at RSUP Dr. Sardjito Yogyakarta. Survival analysis with the Kaplan-Meier curve and Cox regression was used to examine the influence of patient factors (age, sex and nutritional status), tumor factors (tumor size, node), therapeutic factors (type of therapy), clinical factors (anemia ) and COP-NLR score for overall survival of stage III-IVB NPC patients. Results : A total of 358 stage III-IVB NPC patients were analyzed in this study. Determination of COP-NLR score using the NLR limit of 4.28 and the number of thrombocytes 496 x 10 to the power 3 /uL. If NLR less than or same with 4.28 and platelet count less than or same with 496 x 10 to the power 3 /uL, the score is 0. If NLR> 4.28 or platelet count > 496 x 10 to the power 3 / uL, the score is 1. If NLR> 4.28 and platelet count > 496 x 10 to the power 3 / uL, the score 2. Two-year overall survival of patients with a score of 0, 1, 2 was 72.2%, 57.3%, and 41.4% (p = 0.002). A higher COP-NLR score (1&2) showed a decrease in the 2-year overall survival rate than lower score (0)with HR 1.887 (95% CI : 1.26-2.81 and p = 0.002). Conclusion : Stage III-IVB NPC patients with COP-NLR score 0 before therapy have a better 2 years overall survival rate than patients with COP-NLR scores 1 and 2.
