FAKTOR PENYEBAB KETIDAKLENGKAPAN PENGISIAN LEMBAR RESUME MEDIS PASIEN RAWAT INAP DI RUMAH SAKIT: SEBUAH LITERATURE REVIEW

Latar Belakang: Dalam berkas rekam medis pasien rawat inap terdapat berbagai lembar formulir, salah satunya adalah lembar resume medis. Ketidaklengkapan pengisian resume medis akan menghambat proses pengelolaan berkas rekam medis dan akan membuat koding diagnosis dan tindakan tidak sesuai dengan pelayanan kesehatan yang diberikan pada pasien. Ketidaklengkapan pengisian resume medis dapat disebabkan berbagai macam faktor penyebab. Tujuan: Untuk mengetahui faktor penyebab ketidaklengkapan pengisian resume medis. Metode: Jenis penelitian yang digunakan adalah literature review. Sumber pengumpulan data penelitian menggunakan Google Scholar. Seleksi relevasi jurnal menggunakan kriteria inklusi dan eklusi. Hasil: Berdasarkan hasil seleksi review penelitian didapatkan 5 jurnal yang relevan dengan topik penelitian terkait faktor penyebab ketidaklengkapan pengisian lembar resume medis. Hasilnya menunjukkan angka kelengkapan pengisian resume medis belum ada yang mencapai standar minimal kelengkapan. Berbagai macam faktor yang dapat menyebabkan ketidaklengkapan dalam pengisian lembar resume medis diantaranya kurangnya kepatuhan dokter, belum adanya SPO terkait pengisian yang lengkap, kesibukan dokter, kurangnya sosialisasi terkait pengisian resume medis yang lengkap, dan belum adanya kebijakan reward serta punishment. Kesimpulan: Ketidaklengkapan pengisian resume medis disebabkan oleh banyak faktor. Oleh karena itu, petugas harus meningkatkan koordinasi dan komunikasi antar tenaga kesehatan.

Background: There were some forms that could be found in inpatient medical record. One of those was inpatient resume. Incompleteness in resume documentation would obstruct medical record management. This case also might lead inappropriate dignoses and procedures coding. Incompleteness in this documentation could be generated by some factors. Objective: To know the causing factors of incompleteness in resume documentation. Methods: The type of research that was used literature review. Sources of research data collection used Google Scholar. The relevance of journals was used inclusion and exclusion criteria. Results: Based on the results of the research review selection, there were 5 journals relevant to the research topic related to the factors causing the incomplete filling of the medical resume sheet. The result showed that the number of completeness of filling out a medical resume has not yet reached the minimum standard of completeness. Various factors that can cause incompleteness in filling out medical resume sheets include the lack of doctor compliance, the absence of Standard Operational Procedure related to complete filling, busy doctors, lack of socialization related to filling out complete medical resumes, and the absence of a reward and punishment policy. Conclusion: Incomplete filling of medical resumes is caused by many factors. Therefore, officers must improve coordination and communication between health workers.
