Background: Co-assistant activities for Medical Professional Education Program at FKKMK UGM currently still use conventional logbooks. Cases documented in the logbook cannot be used further, not recorded completely, difficult to reanalyze, and require great effort to re-copy the contents of the medical record. Therefore, it is necessary to develop electronic medical records for Co-assistant activities for FKKMK UGM students. Objective: To produce an electronic medical record prototype for co-assistant named Electronic Medical Record for Student (EMRS) at Pediatrics Departmen, Faculty of Medicine, Public Health, and Nursing (FKKMK) UGM. Methods: This type of research was conducted using Research and Development by applying the Systems Development Life Cycle (SDLC) framework to the testing stage. The subjects in this design involved 3 representatives from Pediatrics Department, 5 representatives from FKKMK UGM, and 25 Co-assistant Students at the Child's Station. Data collection techniques were carried out by means of a documentation study, FGD (Focus Group Discussion), and an open questionnaire. The documentation study was carried out at the needs analysis stage, while the FGD and open questionnaire were carried out at the EMRS prototype trial stage. Results: The EMRS prototype was developed on a web-based basis and implemented with the Laravel framework. The EMRS prototype for student access rights consists of 7 main menus, namely: homepage, logbook, patient, explore, repository, achievement, and profile. Meanwhile, the EMRS prototype for admin access rights consists of 3 main menus, namely: dashboard, users, and profiles. Conclusion: The EMRS prototype can be accessed through the address https://emrfk.technosmart.id/. The EMRS prototype has been piloted to users and received feedback in the form of suggestions for EMRS improvements. Keywords: Prototype, Electronic Medical Record, Co-assistant
