Gambaran Peran Apoteker dalam Pengatasan Drug Related Problems dan Persepsi Apoteker terhadap Medication Therapy Management pada Pasien HIV/AIDS di Rumah Sakit Daerah Istimewa Yogyakarta: Narrative Review dan Penelitian Kualitatif

HIV/AIDS merupakan penyakit infeksi sistem imun yang tidak jarang menimbulkan infeksi oportunistik sehingga memerlukan pelayanan kefarmasian yang optimal. Salah satu pelayanan kefarmasian yang sedang dikembangkan adalah Medication Therapy Management (MTM) yang melibatkan kolaborasi tenaga kesehatan untuk mengoptimalkan pengobatan. Penelitian ini bertujuan untuk mengetahui gambaran peran apoteker, drug related problems (DRPs), pengatasan DRPs, dan persepsi apoteker terhadap pelaksanaan MTM dalam pengatasan DRPs pasien HIV/AIDS. Penelitian ini menggunakan metode narative review dan kualitatif. Metode narrative review digunakan untuk mengetahui peran apoteker dan DRPs pasien HIV/AIDS menggunakan database Google Scholar, Pubmed, dan Scopus. Metode kualitatif digunakan untuk mengetahui peran apoteker, DRPs, pengatasan DRPs, dan persepsi apoteker terhadap pelaksanaan MTM dalam pengatasan DRPs pasien HIV/AIDS di rumah sakit Daerah Istimewa Yogyakarta melalui wawancara 5 apoteker yang ditentukan dengan purposive sampling sesuai kriteria inklusi dan eksklusi. Penelitian ini dilakukan pada Desember 2020-April 2021. Hasil narrative review 39 artikel menunjukkan peran apoteker dalam pelayanan HIV/AIDS di rumah sakit yaitu konseling, kolaborasi dengan tenaga kesehatan, mengidentifikasi dan mengatasi DRPs, visite, monitoring efektivitas, keamanan, dan kepatuhan pengobatan, rekonsiliasi, review pengobatan dan riwayat pengobatan sedangkan DRPs yang umum pada pasien HIV/AIDS yaitu dosis tidak tepat, pasien memerlukan tambahan obat, pasien mengalami Adverse Drug Reaction, obat salah, ketidakpatuhan, dan pasien mendapatkan obat tidak perlu. Hasil wawancara lima responden mengungkapkan sebagian besar kesembilan peran hasil narrative review telah dilaksanakan dan pengatasan DRPs pasien HIV/AIDS yang dilakukan yaitu berkomunikasi dengan tenaga kesehatan lain. Selain itu, pelayanan MTM berdasarkan persepsi apoteker dapat diterapkan dan bermanfaat dalam mengatasi DRPs pasien HIV/AIDS di rumah sakit.

HIV/AIDS is an infectious disease of the immune system that common causes opportunistic infections so requires optimal pharmaceutical care. One of the pharmaceutical care that is being developed is Medication Therapy Management (MTM) which involves the collaboration of healthcare professional to optimize medication. The aim of this study is to know the role of pharmacists, drug related problems (DRPs), solving DRPs, and the pharmacists perception for the implementation of MTM in solving DRPs of HIV/AIDS patients. This study uses narrative review and qualitative methods. The narrative review method is used to know the role of pharmacist and DRPs HIV/AIDS patients using Google Scholar, Pubmed, and Scopus databases. The descriptive qualitative method is used to know the role of pharmacist, DRPs, solving DRPs, and the pharmacists perception for the implementation of MTM in solving DRPs of HIV/AIDS patients at hospitals in The Special Region of Yogyakarta through interviews with five pharmacists who are determined by purposive sampling based on inclusion and exclusion criteria. This study was conducted in December 2020-April 2021. The narrative review result of 39 articles show the role of pharmacists in HIV/AIDS care in hospitals are counselling, collaboration with professional healthcare, identifying and solving DRPs, visite, monitoring effectiveness, safety, and adherence of medication, reconciliation, review of medication and medication history while DRPs are common in HIV/AIDS patients are unappropriate dose, patients need additional drug, patients have Adverse Drug Reaction, wrong drug, uncompliance, and patients get unnecessary drug. The interview result of five respondents show that most of nine roles of narrative review result have been carried out and the solving of DRPs in HIV/AIDS patients that has been carried out by communicating with professional healthcare. Moreover, MTM service based on the pharmacists perception can be implemented and useful in solving DRPs of HIV/AIDS patients in hospitals.
