Gambaran Peran Apoteker dalam Pengatasan Drug-Related Problems dan Persepsi Apoteker Terhadap Implementasi Medication Therapy Management pada Pasien Hipertensi di Puskesmas: Narrative Review dan Penelitian Kualitatif

Hipertensi merupakan salah satu penyakit kronis yang memiliki prevalensi cukup tinggi di Indonesia. Dengan prevalensi yang tinggi ini, masih banyak kejadian permasalahan terkait pengobatan atau Drug-Related Problems (DRPs). Salah satu solusi untuk mengatasi DRPs tersebut adalah dengan pelayanan Medication Therapy Management (MTM). Penelitian ini bertujuan untuk mengetahui peran apoteker, DRPs yang sering terjadi melalui narrative review dan wawancara, lalu untuk pengatasan DRPs, dan juga persepsi apoteker tentang penerapan pelayanan MTM dalam mencegah terjadinya DRPs pada pasien hipertensi di Puskesmas Daerah Istimewa Yogyakarta akan didapatkan melalui wawancara. Penelitian ini merupakan penelitian gabungan narrative review dan penelitian kualitatif dengan pendekatan studi kasus dan proses pengambilan data melalui metode wawancara. Penelitian ini akan dilakukan di Puskesmas Daerah Istimewa Yogyakarta dengan responden yang dipilih merupakan apoteker yang bekerja di Puskesmas tersebut dan penentuan jumlah responden menggunakan metode purposive sampling. Data yang dikumpulkan akan dianalisis dan diolah menjadi data deskriptif. Hasil dari narrative review didapatkan 21 artikel yang dikaji dan untuk hasil wawancara yang dilakukan kepada 5 responden apoteker yaitu didapatkan 15 peran apoteker yang dapat meningkatkan luaran terapi pasien hipertensi. Selain itu, diperoleh 3 kejadian DRPs yang sering terjadi pada pasien hipertensi di Puskesmas yaitu salah obat, reaksi obat yang merugikan, dan ketidakpatuhan. Dimana hal yang sama juga diperoleh dari hasil wawancara. Apoteker memiliki persepsi tinggi terhadap proses implementasi MTM untuk mengatasi DRPs yang terjadi. Dalam proses implementasinya memang dapat ditemukan beberapa hambatan tetapi dengan adanya strategi yang cocok dapat menjadikan program MTM lebih baik lagi sehingga dapat meningkatkan luaran terapi pasien hipertensi.

Hypertension is a chronic disease that has a fairly high prevalence in Indonesia. With this high prevalence, there are still many occurrences of drug-related problems (DRPs). One solution to overcome these DRPs is with Medication Therapy Management (MTM) services. This study aims to determine the role of pharmacists, DRPs that often occur through narrative reviews and interviews, then to overcome DRPs, as well as pharmacists' perceptions about the application of MTM services in preventing DRPs in hypertensive patients at the Public Health Center in the Special Region of Yogyakarta which will be obtained through interviews. This research is a combination of narrative review and qualitative research with a case study approach and data collection process through interview method. This research will be conducted at the Yogyakarta Special Region Public Health Center with the selected respondents being pharmacists who work at the Health Center and determining the number of respondents using the purposive sampling method. The data collected will be analyzed and processed into descriptive data. The results of the narrative review obtained 21 articles that were reviewed and for the results of interviews conducted to 5 pharmacist respondents, there were 15 roles of pharmacists who could improve the therapeutic outcomes of hypertensive patients. In addition, there were 3 incidents of DRPs that often occurred in hypertensive patients at the Public Health Center, namely wrong drug selection, adverse drug reactions, and non-compliance. Also, the same thing was obtained from the results of the interviews. Pharmacists have a high perception of the MTM implementation process to overcome the DRPs that occur. In the implementation process, some obstacles can be found, but with a suitable strategy, the MTM program can be even better so that it can improve the therapeutic outcome of hypertensive patients.
