LITERATURE REVIEW : ANALISIS FAKTOR PENYEBAB DAN CARA PENANGGULANGAN TERJADINYA MISFILE DI PUSKESMAS

Latar belakang: Berdasarkan penelitian yang telah dilakukan di beberapa literatur masih ditemukan kejadian misfile berkas rekam medis di puskesmas yang menghambat pelayanan kepada pasien karena lamanya petugas filing dalam mencari berkas. Hal ini dapat mempengaruhi mutu pelayanan puskesmas karena waktu pengadaan berkas rekam medis bertentangan dengan SPM. Akibat lain dari terjadinya misfile berkas rekam medis yaitu petugas pendaftaran membuat rekam medis baru untuk pasien sehingga informasi pasien tersebut tidak berkelanjutan. Tujuan: Mereview literatur terkait kejadian misfile di puskesmas serta faktor yang dapat menyebabkan misfile berkas rekam medis berdasarkan faktor 5m dan upaya yang dilakukan petugas saat terjadi misfile. Metode: Penelitian ini menggunakan metode literature review dengan pendekatan kualitatif. Pencarian artikel terkait melalui Repository UGM, Google Scholar, dan ResearchGate. Kriteria inklusi yang digunakan: (1) artikel/jurnal dengan publikasi dalam rentang 2011-2021, (2) relevan dengan topik penelitian, (3) naskah didapat secara fulltext. Untuk kriteria eksklusi: (1) artikel/jurnal tidak dapat diakses, (2) artikel/jurnal terkait misfile di rumah sakit, (3) jurnal/artikel perancangan dan pengabdian. Hasil: Berdasarkan pencarian literatur dari tiga database, ditemukan 72 literatur untuk dilakukan screening dan mendapatkan delapan literatur yang memenuhi kriteria untuk dianalisis. Penyebab misfile berdasarkan aspek man adalah latar belakang petugas non DIII RMIK, tidak ada pelatihan, dan kurangnya ketilitian dan kedisiplinan. Berdasarkan aspek materials disebabkan berkas yang tidak diberi map, tidak ada tracer, dan tidak diterapkan kode warna. Berdasarkan aspek methods disebabkan tidak ada dan tidak diterapkannya SPO, sistem penjajaran menggunakan SNF, dan tidak digunakannya tracer. Berdasarkan aspek machines disebabkan rak penuh, tidak ada buku ekspedisi. Berdasarkan aspek money dikarenakan kurangnya pendanaan. Kesimpulan: Faktor penyebab misfile dapat disebabkan oleh lima faktor yaitu man, materials, methods, machines, dan money. Upaya telah dilakukan untuk menangani misfile, tetapi dalam beberapa literatur misfile masih belum teratasi akibat kesibukan petugas karena double job.

Background: Based on research that has been conducted in several literatures, there are still incidents of misfiles of medical record files at health centers that hamper service to patients due to the length of time filing officers search for files. This can influence the standard of puskesmas services because the timing of the procurement of medical record files is contrary to the SPM. Another consequence of the misfile of medical record files is that registration officers create new medical records for patients so that the patient information is not sustainable. Objective: Reviewing the literature related to misfile events at the puskesmas and the factors that can cause misfiles of medical record files based on the 5m factor and the efforts made by officers when misfiles occur. Method: This study used a literature review method with a qualitative approach. Search for related articles through the UGM Repository, Google Scholar, and ResearchGate. The inclusion criteria used were: (1) articles/journals with publications in the range 2011-2021, (2) relevant to the research topic, (3) the manuscript was obtained in full text. For exclusion criteria: (1) inaccessible articles/journals, (2) articles/journals related to misfiles in hospitals, (3) design and devotion journals/articles. Result: Based on a literature search from three databases, 72 literatures were found for screening and eight literatures that met the criteria for analysis were found. The causes of misfiles based on the man aspect are the background of non-DIII RMIK officers, no training, and lack of thoroughness and discipline. Based on the materials aspect, the files were not given a map, there was no tracer, and the color code was not applied. Based on the methods aspect, it is caused by the absence and implementation of SPO, the alignment system uses SNF, and the tracer is not used. Based on the machine aspect due to full shelves, there are no expedition books. Based on the money aspect due to lack of funding. Conclusion: The causes of misfiles can be caused by five factors, namely man, materials, methods, machines, and money. Efforts have been made to deal with misfiles, but in some literature misfiles have not been resolved due to the busyness of the officers due to double jobs.
