PERBEDAAN EFEKTIVITAS IRIGASI PROPOLIS 10% DAN PROPOLIS 20% PASCA SCALING DAN ROOT PLANING TERHADAP PENYEMBUHAN JARINGAN PERIODONTAL PADA PASIEN PERIODONTITIS KRONIS (Kajian pada Pocket Depth, Relative Attachment Level, dan ekspresi Interleukin1β)

2021 | Tesis-Spesialis | PERIODONSIAPeriodontitis merupakan satu penyakit inflamasi kronis melibatkan bakteri plak yang mengawali inflamasi lokal pada periodontal ditandai dengan pembengkakan, inflitrasi sel leukosit, dan pelepasan mediator inflamasi serta menyebabkan poket periodontal, berkurangnya pelekatan jaringan, dan resorpsi tulang alveolar yang selanjutnya bisa terjadi kegoyahan gigi. Pada kasus periodontitis, scaling dan root planing tidak dapat dipisahkan. Propolis memiliki sifat anti inflamasi, imunodulator, antioksidan, dan antibakteri. Penelitian ini dilakukan dengan tujuan untuk mengkaji efektivitas irigasi propolis 10% dan propolis 20% pasca scaling dan root planing dalam mempengaruhi Pocket Depth, Clinical Attachment Loss, dan ekspresi IL-1β terhadap penyembuhan jaringan periodontal pada pasien periodontitis kronis. Metode pada penelitian sebanyak 12 elemen gigi didapatkan dari pasien periodontitis kronis yang datang ke RSGM Prof Soedomo serta telah memenuhi kriteria inklusi dan ekslusi. Pasien diukur PPD dan RAL serta diambil cairan sulcus gingiva untuk melihat ekspresi IL-1β sebelum dilakukan tindakan scalling dan root planning. Setelah tindakan scalling dan root planning dilakukan irigasi propolis 10% dan propolis 20%. Pada hari ke-21 dilakukan pengukuran kembali PPD dan RAL serta diambil cairan sulcus gingiva untuk melihat ekspresi IL-1β. Data hasil penelitian dianalisis menggunakan uji shapiro-Wilk dan dilanjutkan dengan Independent T tes. Hasil penelitian dilihat dari uji T-tes menunjukkan scaling dan root planing ditambah irigasi propolis 10% memiliki perbedaan yang bermakna dibandingkan kelompok propolis 20% (ÏË‚0,05). Kesimpulan yang didapatkan dari penelitian ini scaling dan root planing ditambah irigasi propolis 10% menunjukkan kemampuan yang lebih baik dalam mempengaruhi reduksi Periodontal Pocket Depth, Clinical Attachment Loss, dan ekspresi IL-1β terhadap penyembuhan jaringan periodontal pada pasien periodontitis kronis dibandingkan scaling dan root planing ditambah irigasi propolis 20%.
Periodontitis is a chronic inflammatory disease involving plaque bacteria which initiates local periodontal inflammation characterized by swelling, infiltration of leukocyte cells, and release of inflammatory mediators and causes periodontal pockets, reduced tissue adhesion, and resorption of alveolar bone which can lead to tooth shaking. In the case of periodontitis, scaling and root planning cannot be separated. Propolis has anti-inflammatory, immunomodulatory, antioxidant and antibacterial properties. This study was conducted with the objective of assessing the effectiveness of propolis 10% irrigation and 20% propolis irrigation after scaling and root planning in influencing Pocket Depth, Clinical Attachment Loss, and IL-1β expression on periodontal tissue healing in chronic periodontitis patients. The method in this study consisted of 12 dental elements obtained from chronic periodontitis patients who came to the Prof. Soedomo Dental Hospital and had met the inclusion and exclusion criteria. Patients measured for PPD and RAL and gingival sulcus fluid was taken to see IL-1β expression before scaling and root planning. After the scaling and root planning action, 10% and 20% propolis irrigation was performed. On the 21st day, PPD and RAL were measured again and gingival sulcus fluid was taken to see IL-1β expression. The research data were analyzed using the Shapiro-Wilk and continued with the Independent Sample T test. The results of the study seen from the Independent Sample T test showed that scaling and root planning plus 10% propolis irrigation had a significant difference compared to the propolis group 20% (ÏË‚0.05). The conclusions obtained from this study were scaling and root planning plus 10% propolis irrigation showed a better ability to influence Periodontal Pocket Depth, Clinical Attachment Loss, and IL-1β expression on periodontal tissue healing in chronic periodontitis patients than scaling and root planning plus irrigation. propolis 20%.
