KORELASI ANTARA KADAR ANTI dsDNA DENGAN NILAI MEAN PLATELET VOLUME PADA PENDERITA LUPUS ERITEMATOSUS SISTEMIK

Latar Bealakang: Gangguan utama yang terjadi pada lupus eritematosus sistemik (LES) adalah pembentukan autoantibodi. Antibodianti-double stranded DNA (antidsDNA) dijumpai pada lebih dari 75%. Titer antibodi anti dsDNA bervariasi sesuai waktu dan aktivitas penyakit LES. Aktivasi sistem trombosit adalah salah satu peristiwa penting dalam perkembangan aktivitas penyakit LES. Volume trombosit yang merupakan penanda fungsi dan aktivasi trombosit, diukur sebagai mean platelet volume (MPV).Sehingga diperlukan penelitian untuk mengetahui apakah terdapat korelasi antara kadar anti dsDNA dengan nilai MPV untuk menilai aktivitas penyakit LES. Tujuan penelitian ini adalah untuk mengetahui korelasi antara kadar anti dsDNA dengan nilai MPV pada penderita lupus eritematosus sistemik Metode Penelitian deskriptif analitik dengan desain cross sectional. Teknik pengambilan sampel menggunakan consecutive sampling. Data diambil dari catatan medik pasien rawat jalan RSUP dr.Sardjito tahun 2018 sesuai kriteria inklusi dan eksklusi,data diolah secara deskriptif untuk mendapatkan karakteristik subjek penelitian. Korelasi kadar anti dsDNA dengan nilai MPV pada penelitian ini dianalisis menggunakan uji korelasi Pearson atau Spearmans (jika distribusi data tidak normal). Nilai p < 0,05 akan dianggap bermakna secara statistik. Hasil:Dari 62 subjek pasien LES yang terlibat dalam penelitian ini terdiri dari 96,8% perempuan (60 sampel) dan 3,2% laki-laki, dengan manifestasi awal tersering adalah arthritis (80,6%) diikuti dengan mukokutan (37,1% - 54,8%). Hasil analisis korelasi spearman menunjukkan tidak terdapat korelasi yang signifikan antara kadar anti-dsDNA dengan nilai MPV (r= 0,35,p= 0,787) pada pasien LES. Analisis hubungan karakteristik subjek dengan nilai MPV didapatkan hubungan yang bermakna antara MPV dengan lama sakit (p=0,015), dengan leukosit (p=0,017) dan dengan trombositosis (p=0,004). Analisa multivariat variabel yang signifikan terhadap MPV tersebut menunjukkan hanya trombositosis yang bermakna (p=0,004). Kesimpulan: Hasil pada penelitian ini disimpulkan tidak didapatkan korelasi antara kadar anti dsDNA dengan nilai MPV pada penderita lupus eritematosus sistemik.

Background and Objective: The main disorder that occurs in systemic lupus erythematosus (SLE) is the formation of autoantibodies.Anti-double stranded DNA (anti dsDNA) antibodies were found in more than 75%. Anti dsDNA antibody level varies with time and disease activity. Platelet activation is one of the important events in the development of SLE disease activity. Platelet volume, which is a marker of platelet function and activation, is measured as mean platelet volume (MPV). Study is needed to determine whether there is a correlation between anti dsDNA levels and MPV values to assess SLE disease activity. This study aimed to determinate whether there is a correlation between anti dsDNA levels and MPV values in people with systemic lupus erythematosus. Methods; Analytical descriptive research with cross sectional design. With the sampling technique using consecutive sampling. The data taken from the medical records of outpatients at Sardjito Hospital in 2018 according to the inclusion and exclusion criteria. The data were processed descriptively to obtain the characteristics of the research subjects. The correlation of anti dsDNA levels with the MPV value in this study was analyzed using Pearson's or Spearman's correlation test (if the data distribution was abnormal). A p value <0,05 would be considered statistically significant. Results: Of the 62 SLE patients who were involved in this study, 96,8% were female (60 samples) and 3.2% male, with the most common initial manifestation were arthritis (80,6%) followed by mucocutaneous (37,1%- 54,8%). The results of the Spearman correlation analysis showed that there was no significant correlation between anti-dsDNA levels and the MPV value (r = 0,35, p = 0,787) in LES patients. Analysis of the relationship between subject characteristics and MPV value showed a significant relationship between MPV and duration of illness (p = 0,015), with leukocytes (p = 0,017) and with thrombocytosis (p = 0,004). Multivariate analysis from significant variables showed that only thrombocytosis (p = 0.004) showed significant relationship with MPV values. Conclusion:The results of this study concluded that there was no correlation between anti dsDNA levels and MPV values in patients with SLE.
