PERBANDINGAN EFEKTIVITAS KOMBINASI GABAPENTIN - ONDANSETRON DENGAN DEXAMETHASONE - ONDANSETRON SEBAGAI PROFILAKSIS POST OPERATIVE NAUSEA-VOMITING (PONV) PADA OPERASI LAPAROSKOPI

Latar Belakang: Post Operative Nausea and Vomiting (PONV) merupakan komplikasi yang paling sering muncul pada pasien yang menjalani operasi dengan anestesi umum, dengan angka kejadian 40-90%. Meskipun PONV secara umum adalah komplikasi yang dapat hilang sendiri (self-limited), namun dapat menyebabkan komplikasi yang jarang dan berbahaya terhadap pasien yang dapat menambah biaya perawatan secara keseluruhan dengan signifikan. Guideline kombinasi obat untuk mencegah PONV yang ada belum efektif menurunkan kejadian PONV secara signifikan. Adanya beberapa hasil penelitian yang memunculkan kombinasi Gabapentin-Ondansetron efektif dalam menurunkan kejadian PONV. Tujuan: Penelitian ini bertujuan untuk mengetahui perbandingan efektifitas kombinasi Gabapentin-Ondansetron dengan Dexamethasone-Ondansetron sebagai profilaksis Post Operative Nausea and Vomiting (PONV) pada operasi laparoskopi. Metode Penelitian: Double Blind Randomized Controlled Trial/RCT ini melibatkan 78 pasien yang menjalani prosedur laparoskopi. Subjek dibagi menjadi dua kelompok A dan B. Kelompok A diberikan kombinasi Gabapentin 600 mg dan Ondansetron 0.1 mg/KgBB. Kelompok B diberikan kombinasi Dexamethasone 10 mg dan Ondansetron 0.1 mg/KgBB. Pada kedua kelompok ini dilakukan penilaian terhadap kejadian PONV dengan menggunakan VNRS untuk nausea dan dinilai ada atau tidaknya vomiting. Hasil: Pada menit ke 15 dan 30 paska operatif, Gabapentin-Ondansetron lebih baik dibandingkan Dexamethasone-Ondansetron sebagai profilaksis nausea (p < 0,05) pada operasi laparoskopi. Pada menit ke 60, 90 dan 24 jam paska operasi, kombinasi obat kedua kelompok sama baiknya sebagai profilaksis nausea (p > 0,05) pada operasi laparoskopi. Kedua kombinasi obat ini memiliki efek yang sama baiknya sebagai profilaksis vomiting (p > 0,05) paska operatif laparoskopi. Kesimpulan: Kombinasi Gabapentin-Ondansetron lebih efektif daripada kombinasi Dexamethasone-Ondansetron sebagai profilaksis Post Operative Nausea and Vomiting (PONV) pada operasi laparoskopi.

Background: Post Operative Nausea and Vomiting (PONV) is the most common complication in patients undergoing surgery under general anesthesia, with an incidence rate of 40-90%. Although PONV in general is a self-limited complication, it can cause rare and dangerous complications for patients which can significantly increase overall treatment costs. Drug combination guidelines for preventing existing PONV have not been effective in significantly reducing the incidence of PONV. The existence of several research results which gave rise to the Gabapentin-Ondansetron combination was effective in reducing the incidence of PONV. Objective: This study aims to compare the effectiveness of the combination of Gabapentin-Ondansetron with Dexamethasone-Ondansetron as a Post Operative Nausea and Vomiting (PONV) prophylaxis in laparoscopic surgery. Methods: This Double Blind Randomized Controlled Trial / RCT involved 78 patients who underwent a laparoscopic procedure. Subjects were divided into two groups A and B. Group A was given a combination of Gabapentin 600 mg and Ondansetron 0.1 mg / KgBW. Group B was given a combination of Dexamethasone 10 mg and Ondansetron 0.1 mg / KgBW. In both groups, an assessment of the incidence of PONV was carried out using VNRS for nausea and whether or not vomiting is present. Result: At 15 and 30 minutes postoperatively, the drug combination in group A was better than the drug combination in group B for nausea prophylaxis (p <0.05) in laparoscopic surgery. At 60, 90 and 24 hours postoperatively, the drug combination of the two groups was as good as nausea prophylaxis (p> 0.05) in laparoscopic surgery. The two drug combinations had the same effect as vomiting prophylaxis (p> 0.05) after laparoscopic surgery. Conclusion: Gabapentin-Ondansetron combination is more effective than the Dexamethasone-Ondansetron combination as a Post Operative Nausea and Vomiting (PONV) prophylaxis in laparoscopic surgery.
