PENGARUH EOSINOFIL DAN LIMFOSIT PADA GAMBARAN HISTOPATOLOGIS TERHADAP LUARAN FUNGSIONAL PASIEN HIRSCHSPRUNG PASCA OPERASI DUHAMEL DI RSUP DR. SARDJITO

Latar Belakang : Penyakit Hirschsprung (HSCR) adalah gangguan migrasi pada sistem saraf intestinal yang ditandai dengan tidak adanya sel ganglion pada kolon. Salah satu manajemen definitive HSCR adalah dengan metode operasi Duhamel. Luaran fungsional pasien pasca operasi dinilai untuk mengetahui keberhasilan operasi. Luaran fungsional pasien dapat dipengaruhi oleh beberapa hal, salah satunya adalah kondisi eosinofilia dan limfositosis mukosa pada biopsi preoperasi. Hanya terdapat satu penelitian mengenai pengaruh eosinofil mukosa pada luaran fungsional pasien, namun belum ada yang meneliti pengaruh eosinofil dan limfosit mukosa pada luaran fungsional pasien HSRC pasca operasi. Tujuan : Untuk mengetahui pengaruh eosinofil dan limfosit pada gambaran histopatologis terhadap luaran fungsional pasien Hirschsprung pasca operasi Duhamel di RSUP Dr. Sardjito. Metode : Penelitian ini merupakan penelitian observasional dengan rancangan studi cross sectional yang dilakukan pada 41 pasien Duhamel yang menjalani operasi sejak Januari 2014 hingga Oktober 2020 di RSUP Dr. Sardjito. Data diambil dari rekam medis dan hasil biopsi preoperasi. Hasil dianalisis dengan uji bivariat Fisher Exact Test. Hasil : Luaran fungsional pasien HSCR pasca operasi dinilai menggunakan klasifikasi Krickenbeck, yaitu voluntary bowel movement, soiling, dan konstipasi. Kondisi eosinofilia mukosa (>20 sel/high power field) tidak menunjukkan pengaruh yang signifikan terhadap luaran fungional pasien. Kondisi limfositosis mukosa (>10/100 sel epitel) memiliki pengaruh yang signifikan terhadap soiling (p=0,006*) dan konstipasi (p=0.031*). Luaran fungsional juga dapat dipenaruhi oleh karakteristik subjek namun hasil analisis tidak menunjukkan perbedaan yang signifikan. Kesimpulan : Eosinofilia mukosa tidak mempengaruhi luaran fungsional pasien HSCR pasca operasi Duhamel. Limfositosis mukosa mempengaruhi luaran fungsional berupa soiling dan konstipasi pada pasien HSRC pasca operasi Duhamel.

Background : Hirschsprung disease (HSCR) is a disease due to migration failure of enteric nervous system, characterized by the absence of ganglion cells in the colon. One of the definitive management is Duhamel procedure. The functional outcome of postoperative patients was assessed to determine the success of the procedure and can be affected by mucosal eosinophilia and lymphocytosis. There is only one study regarding the effect of mucosal eosinophils on the functional outcome of patients, but no one has examined the effect of mucosal eosinophils and lymphocytes on the functional outcome of postoperative HSRC patients. Objective : To determine the effect of eosinophils and lymphocytes on the histopathological picture on the functional outcome of Hirschsprung's patients after Duhamel's surgery at Dr. Sardjito. Methods : This study is an observational study with a cross sectional study design which was conducted on 41 Duhamel patients who underwent surgery from January 2014 to October 2020 at Dr. Sardjito. Data taken from medical records and preoperative biopsy results. The results were analysed by using the bivariate Fisher Exact Test. Results : The functional outcome of postoperative HSCR patients was assessed using the Krickenbeck classification, namely voluntary bowel movement, soiling, and constipation. The condition of mucosal eosinophilia (> 20 cells / high power field) did not show a significant effect on the patient's functional outcome. The condition of mucosal lymphocytosis (> 10/100 epithelial cells) had a significant effect on soiling (p = 0.006*) and constipation (p = 0.031*). The functional output can also be affected by the characteristics of the subject, but the analysis results do not show a significant difference. Conclusion : Mucosal eosinophilia did not affect the functional outcome of Duhamel's postoperative HSCR patients. Mucosa Mucosal lymphocytosis affects the functional outcome of soiling and constipation in post-Duhamel HSRC patients.
