EVALUASI KIFOSIS PASCA TINDAKAN OPEN DOOR LAMINOPLASTY DENGAN TITANIUM MESH PADA PASIEN OSSIFICATION OF THE POSTERIOR LONGITUDINAL LIGAMENT (OPLL) DI RSUP DR SARDJITO YOGYAKARTA

INTISARI Latar Belakang Di Indonesia, banyak pasien OPLL yang datang dengan kondisi mielopati dan telah terjadi perubahan kurva servikal ke arah kifosis berat. Saat ini teknik operasi open door laminoplasty (ODL) yang ada memiliki keterbatasan pada kondisi ini. Berdasarkan hal tersebut di atas maka perlu dicari teknik yang mampu menangani pasien dengan kondisi seperti itu. Modifikasi titanium mesh untuk mempertahankan sisi terbuka pada teknik open door laminoplasty belum pernah dilakukan sebelumnya. Titanium mesh diharapkan dapat menjaga kestabilan tulang sekaligus mengurangi terjadinya perburukan kyphosis yang ada. Evaluasi kifosis pasca operasi dengan metode ini diperlukan untuk membuktikan teknik ini. Metode Penelitian ini adalah penelitian dengan desain penelitian serial kasus pada penderita OPLL di RSUP Dr. Sardjito Yogyakarta yang menjalani tindakan operasi open door laminoplasty dengan menggunakan titanium mesh pada periode Mei 2014 sampai dengan Desember 2018. Evaluasi kifosis dilakukan dengan menggunakan ishihara curvature index. Hasil Pada penelitian ini terdapat 1 peserta perempuan (20%) dan 4 laki � laki 80% dengan usia rata � rata adalah 52,2 + 7,2 tahun. Pasien telah menjalani menjalani operasi rata � rata sejak 43,4 + 27,6 bulan yang lalu. Angka curvature index pra operasi pada penelitian ini terendah adalah -13,70 dan tertinggi adalah 7,28, sementara angka curvature index pasca operasi terendah adalah -12,17 dan tertinggi adalah 5,92, dari data tersebut terdapat tiga pasien mengalami penurunan angka curvature index dan dua pasien lainnya terjadi peningkatan angka curvature index. Kesimpulan Pada penelitian ini disimpulkan bahwa tindakan open door laminoplasty dengan titanium mesh pada pasien ossification of the posterior longitudinal ligament memiliki kecenderungan tidak memperburuk lengkung lordosis. Sehingga teknik ini memiliki prospek untuk mempertahankan lengkung servikal pada pasien OPLL degeneratif dengan lengkung servikal yang sudah kifotik.

ABSTRACT Background In Indonesia, many patients with OPLL present with myelopathy conditions and there has been a change in the cervical curve towards severe kyphosis. Currently the existing Open Door Laminoplasty (ODL) surgery technique has limitations on this condition. Based on the above, it is necessary to look for techniques that are able to manage patients with such conditions. The modification of the titanium mesh to maintain the open side of the open door laminoplasty technique has never been done before. Titanium mesh is expected to maintain bone stability and at the same time reduce the occurrence of worsening of existing kyphosis. Evaluation of postoperative kyphosis by this method is necessary to prove this technique. Method This study was a case series study design for OPLL patients at Dr. Sardjito Yogyakarta, who underwent open door laminoplasty surgery using titanium mesh in the period May 2014 to December 2018. Evaluation of kyphosis was carried out using the Ishihara curvature index. Results In this study there were 1 female participant (20%) and 4 male (80%) with the mean age was 52.2 + 7.2 years. Patients underwent surgery for an average of 43.4 + 27.6 months. The lowest preoperative curvature index number in this study was -13.70 and the highest was 7.28, while the lowest postoperative curvature index number was -12.17 and the highest was 5.92, from these data there were three patients who experienced a decrease in the curvature rate. index and two other patients had an increase in the curvature index. Conclusion In this study, it was concluded that open door laminoplasty with titanium mesh in patients with ossification of the posterior longitudinal ligament did not tend to worsen the lordotic curve. So this technique has the prospect of maintaining cervical curvature in degenerative OPLL patients with kyphotic cervical curvatures.
