OTONOMI PASIEN DALAM ISU TRANSGENDER: STUDI KASUS FILM THE DANISH GIRL DALAM PERSPEKTIF BIOETIKA

Fokus penelitian ini adalah persoalan bioetika terkait layanan kesehatan transgender di dalam Film The Danish Girl. Film The Danish Girl, sebagai salah satu film fenomenal di tahun 2015 menjadi salah satu contoh kasus praktik operasi transgender pertama yang dapat ditilik sebagai basis data yang ada di dalam kehidupan nyata. Pertanyaan utama yang akan berusaha dijawab adalah apakah tindakan medis seperti operasi kelamin pada pasien transgender telah memenuhi prinsip otonomi di dalam bioetika. Serta ingin melihat apa saja aspek yang ada di dalam prinsip otonomi tersebut. Penelitian ini menggunakan metode kajian kepustakaan. Pustaka primer yang digunakan adalah film The Danish Girl oleh Tom Hooper sebagai objek material serta prinsip bioetika oleh Beauchamp & Childress sebagai objek formal. Langkah penelitian ini terdiri dari deskripsi, interpretasi, komparasi, dan induksi. Hasil dari penelitian sebagai berikut: pertama, setiap pengambilan keputusan tindakan medis transgender harus dipastikan bahwa agen rasional sudah terinformasi, atau sudah memiliki informasi yang memadai terkait tindakan medis yang akan dilakukan. Dalam keputusan pelayanan kesehatan, penghormatan seorang dokter terhadap otonomi pasien secara umum akan menyiratkan bahwa pasien memiliki kapasitas untuk bertindak secara sadar, dengan pemahaman, dan tanpa pengaruh orang lain yang akan mengurangi nilai tindakan bebas dan sukarela mereka. Kedua, prinsip otonomi adalah dasar bagi praktik informed consent dalam hubungan dokter-pasien mengenai pelayanan kesehatan terapi atau bedah dalam kasus transeksual. Ketiga, otonomi pasien juga harus disokong dengan dukungan psikologis dan sosial, komunikasi terbuka dengan dokter dan edukasi terkait tindakan medis yang akan dilakukan.

Focus of this research is the issue of bioethics in transgender health care featured in The Danish Girl film. The Danish Girl, as one of the phenomenal films in 2015, is an example of the first case of transgender surgery that can be viewed as data that refers to real life. The main question trying to be answered is whether medical measures such as genital surgery on transgender patients meet the principle of autonomy. As well as looking for other existing aspects that support the principle of autonomy. This research uses literature review method. The primary literature used in this research are The Danish Girl film by Tom Hooper as a material object and bioethical principles discourse by Beauchamp & Childress as a formal object. The research stages consist description, interpretation, comparison, and induction. The results of the study are: first, every decision on transgender medical actions must be ensured that rational agents have been informed, or have sufficient information related to medical actions that will be carried out. In health care decisions, a doctor's respect for patient autonomy generally implies that the patient has the capacity to act intentionally, with understanding, and without other influences which will diminish the value of their free and voluntary action. Second, the principle of autonomy is the basis for the practice of informed consent in the doctor-patient relationship regarding health care or surgery in transsexual cases. Third, patient autonomy must also be supported by psychological and social support, open communication with doctors and education regarding medical actions to be performed.
