Kelengkapan Pengisian Rekam Medik di Klinik Pendidikan Prostodonsia RSGM UGM Prof Soedomo

RSGM UGM Prof Soedomo terdiri atas beberapa klinik pendidikan, salah satunya adalah klinik pendidikan prostodonsia yang wajib menyelenggarakan pengisian rekam medik. Kelengkapan pengisian rekam medik di klinik pendidikan prostodonsia RSGM UGM Prof Soedomo sangat penting untuk menunjang fungsi rekam medik dan proses akreditasi rumah sakit. Tujuan penelitian ini adalah untuk mengetahui persentase kelengkapan pesisian rekam medik di klinik pendidikan prostodonsia RSGM UGM Prof Soedomo pada tahun 2017. Seratus dua puluh rekam medik di klinik pendidikan prostodonsia yang diteliti terdiri atas 60 rekam medik pada pasien GTSL dan 60 rekam medik pasien GTC. Teknik pengambilan sampel menggunakan metode systematic ramdom sampling dan simple random sampling. Rekam medik tersebut dianalisis kelengkapannya menggunakan bantuan lembar kerja berupa checklist yang disusun berdasarkan standar Permenkes No. 269/Menkes/Per/III/2008, Buku Panduan Rekam Medik Kedokteran Gigi yang disusun oleh Kemenkes pada tahun 2014, Instrumen Survei Standar Nasional Akreditasi Rumah Sakit tahun 2017 dan lembar rekam medik RSGM UGM Prof Soedomo. Persentase kelengkapan pengisian lembar rekam medik umum di klinik pendidikan prostodonsia RSGM UGM Prof Soedomo pada tahun 2017 hanya mencapai 27,77%, persentase kelengkapan rekam medik khusus prostodonsia pada pasien GTSL sebesar 71,59%, dan persentase kelengkapan rekam medik khusus prostodonsia pada pasien GTC sebesar 68,71%.

RSGM UGM Prof Soedomo consists of several educational clinics, such as a prosthodontics educational clinic. This clinic is obliged to filling out medical record completely. Completeness of medical records at the prosthodontics educational clinic of RSGM UGM Prof Soedomo is very important to support the function of medical records and also the hospital accreditation process. The purpose of this study was to determine the percentage of completeness of medical record in the prosthodontics educational clinic of RSGM UGM Prof Soedomo in 2017. One hundred and twenty medical records in the prosthodontics educational clinic studied consisted of 60 medical records for removable denture patients and 60 medical records for fixed denture patients. The sampling technique used in this study was systematic random sampling and simple random sampling. Completeness of medical records is analyzed using worksheets in the form of a checklist compiled based on Permenkes No. 269/Menkes/Per/III/2008, Handbook of Dental Medical Records compiled by Ministry of Health in 2014, National Standard Survey Instrument for Hospital Accreditation in 2017 and medical record sheets of RSGM UGM Prof Soedomo. The percentage of completeness of general medical record sheets at prosthodontics educational clinic of RSGM UGM Prof Soedomo in 2017 only reached 27,77%, the percentage of completeness of special medical records for removable denture patients was 71,59%, and the percentage of completeness of special medical records for fixed denture patients was 68,71%.
