Kelengkapan Pengisian Rekam Medik di Klinik Pendidikan Periodonsia RSGM UGM Prof. Soedomo

Rekam medik merupakan berkas yang mengandung informasi tentang perawatan pasien. Rekam medik harus dibuat dengan lengkap sehingga akurat informasinya. Informasi medis dari rekam medik digunakan oleh seluruh petugas kesehatan untuk merawat pasien dan media sarana komunikasi antar petugas kesehatan. Rekam medik yang lengkap juga berpengaruh terhadap proses akreditasi rumah sakit. Standar kelengkapan rekam medik berdasarkan Kepmenkes no 129 tahun 2008 adalah 100% setelah selesai pelayanan selama 24 jam. Masalah pengisan rekam medik yang tidak lengkap banyak terjadi di fasilitas kesehatan. Tujuan dari penelitian ini adalah untuk mengetahui kelengkapan butir-butir isian dari rekam medik klinik pendidikan periodonsia. Jenis penelitian adalah penelitian deskriptif dengan pendekatan kuantitatif menggunakan instrumen penelitian berupa lembar check list. Sampel rekam medik yang digunakan sebanyak 120 rekam medik dengan pengambilan sampel menggunakan metode systematic random sampling. Variabel yang diteliti adalah kelengkapan isi rekam medik klinik pendidikan Periodonsia berdasarkan Permenkes RI no 269 tahun 2008 dengan hasil lengkap dan tidak lengkap. Hasil penelitian kelengkapan isi rekam medik klinik pendidikan Periodonsia yang terdiri dari 10 aitem disajikan dalam bentuk persentase. Persentase kelengkapan variabel identitas sebesar 0%, tanggal dan waktu sebesar 0%, anamnesis sebesar 0%, pemeriksaan fisik dan penunjang medik sebesar 0%, odontogram klinik sebesar 0%, diagnosis sebesar 0%, rencana penatalaksanaan sebesar 17%, tindakan sebesar 0%, persetujuan tindakan sebesar 0%, serta nama dan tanda tangan dokter sebesar 0%. Kesimpulan: Pengisian berkas rekam medik di klinik pendidikan Periodonsia RSGM UGM Prof. Soedomo pada tahun 2017 tidak ada yang lengkap.

Medical record is document of patients care. The content of medical record should be complete, so the information will be accurate. Information of medical record was used by all healthcare providers for patients treatment and media of communication for practitioners. The completeness of medical record will affect hospital accreditation. Standard of medical record completeness based on Kepmenkes no 129/2008 is 100% after 24 hours of patients care completion. Incomplete medical record was a problem occurred in many health facilities. The purpose of this research was to study of the completeness of the content of medical records at periodontics educational clinic. The research was a descriptive study with a quantitative approach using a checklist sheet as research instrument. The samples of the study was 120 medical records. Sampling method was systematic random sampling method. The variables was the completeness of the content of medical record based on Permenkes RI No 269/2008 with the result were complete and incomplete. The result of the study on the completeness of the contents of the Periodontics education clinical medical record consisting of 10 items were presented in percentage form. The percentage of the variables identity (0%), date and time (0%), anamnesis (0%), physical examination (0%), odontogram (0%), diagnosis (0%), treatment planning (17%), therapy (0%), informed consent (0%), and doctors name and signature (0%). This study showed that the completeness of medical record filled was 0%. The completeness of medical record filled based on practitioners gender was 0%. The completeness of medical record filled at Periodontal Clinic of RSGM UGM Prof. Soedomo was incomplete.
