TINJAUAN KESIAPAN KEGIATAN PENYUSUTAN DAN PEMUSNAHAN BERKAS REKAM MEDIS DI PUSKESMAS PENGASIH I KULON PROGO TAHUN 2020

Latar belakang: Puskesmas Pengasih I Kulon Progo belum pernah melaksanakan penyusutan maupun pemusnahan berkas rekam medis. Adanya peningkatan jumlah pasien yang diikuti dengan peningkatan jumlah berkas rekam medis menyebabkan kebutuhan ruang penyimpanan meningkat. Permasalahan tersebut menghambat pengadaan dan pengembalian rekam medis sehingga petugas ingin melakukan penyusutan dan pemusnahan rekam medis. Tujuan: Mengetahui kesiapan penyusutan dan pemusnahan berkas rekam medis di Puskesmas Pengasih I Kulon Progo. Metode: Jenis penelitian yang digunakan adalah penelitian deskriptif dengan pendekatan kualitatif dan menggunakan rancangan penelitian studi kasus. Hasil: Penyusutan rekam medis akan melibatkan petugas pendaftaran, kasir, dan perekam medis. Tidak ada alat, bahan, dan anggaran dana khusus untuk pelaksanaan penyusutan berkas rekam medis. Belum terdapat SPO Penyusutan Rekam Medis, sehingga kepala instalasi rekam medis Puskesmas Pengasih I meminta peneliti untuk merancang SPO tersebut dan hasilnya telah disetujui namun belum ditetapkan oleh kepala puskesmas. Dalam kesiapan kegiatan pemusnahan berkas rekam medis, Puskesmas Pengasih I belum menentukan pihak yang terlibat, alat, bahan yang digunakan, dan anggaran biaya yang dibutuhkan. Peneliti merancang SPO Pemusnahan Rekam Medis dengan arahan dan persetujuan Kepala Instalasi rekam medis dan koordinator UKP (Upaya Kesehatan Perseorangan). Kesimpulan: Kegiatan penyusutan rekam medis di Puskesmas Pengasih I Kulon Progo berdasarkan unsur 5M yaitu man, money, method, machine, dan material siap untuk mulai dilaksanakan. Sedangkan kesiapan pemusnahan rekam medis ditinjau dari unsur 5M belum bisa direalisasikan karena hanya unsur man yang sudah ada, yaitu tim pemusnah yang disyaratkan sesuai dengan aturan, sedangkan unsur money, method, material dan machine belum tersedia.

These problems hampered the procurement and return of medical records so that officers wanted to reduce and destroy medical records. Objective: Knowing the readiness of depreciation and destruction of medical record files at Puskesmas Pengasih I Kulon Progo. Methods: This type of research is a descriptive study with a qualitative approach and a case study research design. Result: Medical record shrinkage will involve registration officer, cashier, and medical recorder. There are no special tools, materials, and budget for the implementation of depreciation of medical record files. There is no SOP for depreciation of medical records, so the head of the Puskesmas Pengasih I medical record installation asked the researcher to design the SOP and the results have been approved but have not been determined by the head of the public health center. In the readiness for the activity to destroy medical record files, Puskesmas Pengasih I has not yet determined the parties involved, the tools, materials used, and the required budget. The researcher designed the Medical Record Destruction SOP with the direction and approval of the Head of the Medical Record Installation and the UKP (Individual Health Effort) coordinator. Conclusion: The depreciation activity of medical records at Puskesmas Pengasih I Kulon Progo based on the 5M elements, namely man, money, method, machine, and material is ready to start. Meanwhile, the readiness to destroy medical records in terms of the 5M element cannot be realized because only the human elements already exist, namely the destruction team that is required according to the rules, while the elements of money, method, material and machine are not yet available.
