PERANCANGAN FORMULIR RINGKASAN MASUK DAN KELUAR DAN MAP REKAM MEDIS PASIEN DI RUMAH SAKIT CONDONG CATUR

ABSTRAK Latar Belakang: Formulir ringakasan masuk dan keluar tidak ada di Rumah Sakit Condong Catur, tetapi fungsinya digantikan oleh formulir lembar rawat inap. Selain itu untuk kelancaran pengoperasionalan formulir diperlukan juga standar prosedur opersional. Map rekam medis pasien Rumah Sakit Condong Catur memerlukan tambahan karena adanya perubahan ke terminal digit filing sehingga memerlukan kode warna yang melekat pada map tersebut. Selain itu map rekam medis pasien ini juga memerlukan beberapa tambahan item yang menempel pada map tersebut seperti catatan khusus supaya disana dapat ditempel beberapa catatan seperti potensial program rujuk balik. Tujuan: Tujuan perancangan ini adalah desain ulang formulir ringkasan masuk dan keluar, map rekam medis pasien yang dilakukan di Rumah Sakit Condong Catur. Metode: Metode dan langkah-langkah dari RCH Forms Commitee (2006) digunakan perancang sebagai acuan dalam membuat formulir dan map. Hasil: Perancangan ini dapat menghasilkan desain akhir dari formulir ringkasan masuk dan keluar dengan tambahan draft standar prosedur opersional pengisiannya dan map rekam medis pasien dengan tambahan kode warna. Kesimpulan: Penelitian ini menghasilkan masing-masing satu desain dari formulir ringkasan masuk dan keluar yang akan di digunakan pada tanggal 1 Agustus 2020 dan map rekam medis pasien serta tabel kode warna digunakan 1 Juli 2021. Saran : Perancang menyarankan agar petugas Rumah Sakit Condong Catur sebelum mengimplementasikan formulir dan map tersebut sebaiknya diuji cobakan dulu. ABSTRACT Background: Entry and exit summary forms are not available at the Rumah Sakit Condong Catur, but their functions are replaced by inpatient forms. In addition to the smooth operation of the forms, standard opersional prosedur is also required. Rumah Sakit Condong Catur patient medical record folder requires additional because of a change to TDF so it requires a color code attached to the map. Besides this patient's medical record folder also requires some additional items attached to the folder such as special notes so that there can be attached a number of records such as potential referral program. Purpose: The purpose of this design is to design the entry and exit summary form, the patient's medical record folder which is carried out at the Condong Chess Hospital. Method: The methods and steps of the RCH Forms Committee (2006) are used by the designer as a reference in making forms and maps. Results: This design can produce the final design of the entry and exit summary form with the addition of the filling standard opersional prosedur draft and the patient's medical record folder with additional color coding. Conclusion: This study resulted in each of the designs of the entry and exit summary forms to be used on August 1, 2020 and the patient medical record map and color code table used on July 1, 2021. Suggestion : The designer suggested that Rumah Sakit Condong Catur officers prior to implementing the forms and maps it should be tested first.
