ANALISIS KELENGKAPAN PENGISIAN FORMULIR RESUME MEDIS BERDASARKAN STANDAR MIRM 15 STANDAR NASIONAL AKREDITASI RUMAH SAKIT (SNARS) DI RSUD KABUPATEN TEMANGGUNG

Latar Belakang : Resume medis merupakan formulir yang sangat penting dalam formulir rawat inap, dimana kelengkapan isinya menjadi tanggung jawab semua pihak yang terlibat dalam pengisian resume medis tersebut. Ketidaklengkapan resume medis akan berdampak pada hasil akreditasi. Resume medis ini terdapat pada SNARS standar MIRM 15 yang memiliki enam kriteria. RSUD Kabupaten Temanggung pernah melakukan akreditasi KARS pada tahun 2017 dan saat ini sedang mempersiapkan akreditasi SNARS untuk yang pertama kali. Tujuan : Penelitian ini bertujuan untuk mengetahui presentase kelengkapan, faktor-faktor penyebab ketidaklengkapan, upaya pengendalian ketidaklengkapan, dan upaya-upaya yang telah dilakukan untuk mengatasi ketidaklengkapan resume medis oleh petugas rekam medis berdasarkan elemen penilaian MIRM 15 SNARS. Metode : Jenis penelitian yang digunakan dalam penelitian ini yaitu penelitian kuantitatif yang digunakan untuk melihat presentase kelengkapan pengisian formulir resume medis serta digunakan penelitian deskriptif dengan pendekatan kualitatif untuk melihat faktor penyebab dan upaya pengendalian ketidaklengkapan pengisian formulir resume medis. Teknik dalam pengambilan data menggunakan wawancara, studi dokumentasi, dan observasi. Penelitian ini dilakukan di RSUD Kabupaten Temanggung pada bulan Februari hingga Juni 2020. Hasil : Pengisian resume medis di RSUD Kabupaten Temanggung masih belum lengkap. Hasil kelengkapan rata-rata berdasarkan SNARS MIRM 15 sebesar 59,85% dengan kelengkapan rata-rata EP 1 94,21%, EP 2 48,95%, EP 3 49,47%, EP 4 36,84%, EP 5 57,89%, dan EP 6 72,10%. Faktor penyebab ketidaklengkapan dari unsur man yaitu DPJP merupakan dokter tamu dan untuk unsur method yaitu kurang terkoordinasikannya waktu pengisian formulir resume medis. Upaya yang dilakukan untuk mengatasi ketidaklengkapan dari unsur man yaitu dilakukannya kegiatan assembling dan analisis kelengkapan pada berkas rekam medis oleh petugas rekam medis serta unsur method yaitu adanya SPO tentang pengisian formulir resume medis. Kesimpulan : Kelengkapan formulir resume medis masih belum sepenuhnya terisi lengkap. Faktor penyebab dan upaya untuk mengatasi ketidaklengkapan meliputi unsur man dan methods. Saran kepada petugas assembling agar memanggil kepala bangsal apabila terdapat ketidaklengkapan pada resume medis. Kata Kunci : Analisis Kelengkapan, Resume Medis, SNARS.

Background : Discharge summary is a very important form in the inpatient form, where the completeness of its contents is the responsibility of all parties involved in filling out the discharge summary. Incomplete discharge summary will have an impact on the result of accreditation. This discharge summary is available on the standard MIRM 15 SNARS which has six criteria. RSUD Kabupaten Temanggung has done KARS accreditation in 2017 and is currently preparing SNARS accreditation for the first time. Objective : This study aims to determine the percentage of completeness, factors causing incompleteness, efforts to control incompleteness, and efforts that have been made to overcome the incompleteness of discharge summary by medical record officers are based on the element of evaluation MIRM 15 SNARS. Methods : The type of the research that used in this research is quantitative that is used to see the percentage of completeness of filling discharge summary form and used descriptive research with qualitative approach to see the causal factor and efforts to control the incompleteness of filling out discharge summary form. Techniques in data collection using interviews, study documentation, and observation. This research was conducted in RSUD Kabupaten Temanggung in February until June 2020. Results : Filling out discharge summary at RSUD Kabupaten Temanggung is still incomplete. The results of the average completeness based on SNARS MIRM 15 is 59,85% with average completeness of EP 1 94,21%, EP 2 48,95%, EP 3 49,47%, EP 4 36,84%, EP 5 57,89%, and EP 6 72,10%. Causal factors incompleteness of the man element is DPJP is the visiting doctor and for the method element is the lack of coordination in filling out discharge summary form. The efforts made to overcome the incompleteness of the man element is did assembling and completeness analysis of the discharge summary file by the medical record officer and the element of method is the existence of SPO about filling out discharge summary form. Conclusion : Completeness of the discharge summary form is still not fully filled out. Causal factors and efforts to overcome incompleteness include man and methods elements.
