PERANCANGAN FORMULIR RAWAT JALAN, INFORMED CONSENT, DAN RAWAT JALAN GIGI SERTA SOP PENGISIANNYA DI PUSKESMAS CANGKRINGAN SLEMAN

Latar Belakang : Rekam medis yang baik didukung oleh formulir yang memenuhi peraturan dan aspek desain formulir rekam medis, serta SOP pengisian formulir tersebut. Desain formulir yang baik dapat membantu dalam efisiensi pengumpulan dan penyebaran data (IFHIMA, 2018). Puskesmas Cangkringan telah memiliki formulir rekam medis, tetapi belum sesuai peraturan dan aspek desain formulir. Formulir Rawat Jalan, Formulir Informed Consent dan Formulir Rawat Jalan Gigi belum sesuai peraturan dan aspek desain formulir, sehingga masih perlu diperbaiki. Oleh karena itu diperlukan perancangan Formulir Rawat Jalan, Formulir Informed Consent dan Formulir Rawat Jalan Gigi beserta SOP Pengisiannya. Tujuan : Menghasilkan rancangan Formulir Rawat Jalan, Formulir Informed Consent dan Formulir Rawat Jalan Gigi beserta SOP Pengisiannya. Metode : Penelitian ini menggunakan tahapan perancangan yang mengacu pada RCH (2006) tanpa disertai tahap ujicoba formulir. Proses pengumpulan data menggunakan teknik observasi, studi dokumentasi, wawancara dan Focus Group Discussion (FGD). Hasil : Dari perancangan dihasilkan dua alternatif rancangan Formulir Rawat Jalan, Formulir Informed Consent dan Formulir Rawat Jalan Gigi beserta SOP Pengisiannya. Kedua alternatif rancangan tersebut kemudian didiskusikan dan terpilih Formulir Rawat Jalan alternatif satu, Formulir Informed Consent alternatif dua dan Formulir Rawat Jalan Gigi alternatif satu. Kesimpulan : Formulir Rawat Jalan, Formulir Informed Consent dan Formulir Rawat Jalan Gigi di Puskesmas Cangkringan belum sesuai dengan peraturan dan aspek desain formulir, serta belum memiliki SOP khusus mengenai pengisiannya performulir, sehingga perlu dilakukan perancangan ulang terhadap formulir - formulir tersebut dan dilakukan perancangan SOP pengisiannya.

Background: A good medical record is supported by a form that meets the regulations and design aspects of the medical record form, and the form filling SOP. Good form design can help in the efficiency of data collection and dissemination (IFHIMA, 2018). Cangkringan Health Center already has a medical record form, but it does not comply with the regulations and form design aspects. Outpatient Forms, Informed Consent Forms and Dental Outpatient Forms do not comply with regulations and form design aspects, so they still need to be improved. Therefore, it is necessary to design an Outpatient Form, Informed Consent Form and Dental Outpatient Form along with their SOPs. Objective: To produce a draft Outpatient Form, Informed Consent Form and Dental Outpatient Form with their SOP. Method: This study uses a design phase that refers to RCH (2006) without the form trial phase. The process of collecting data uses observation techniques, documentation studies, interviews and Focus Group Discussions (FGD). Results: The design produced two alternative designs for Outpatient Form, Informed Consent Form, Dental Outpatient Form and SOPs. The two alternative designs are then discussed and the chosen designs are alternative one Outpatient Form, alternative two Informed Consent Form and alternative one Dental Outpatient Form. Conclusion: Outpatient Forms, Informed Consent Forms and Dental Outpatient Forms at Cangkringan Health Center are not in accordance with the rules and aspects of form design, and do not yet have a specific SOP regarding filling out the form, so it is necessary to re-design the forms and design the SOP for the filling.
