Kelengkapan Pengisian Rekam Medik di Klinik Unit Pelayanan Umum RSGM UGM Prof. Soedomo Tahun 2017

Latar Belakang: Semua fasilitas pelayanan kesehatan harus memiliki catatan mengenai pasiennya, catatan tersebut adalah dalam bentuk rekam medik. Rekam medik harus diisi secara lengkap karena merupakan salah satu alat komunikasi antar tenaga kesehatan dan sebagai dasar penilaian mutu pelayanan kesehatan. Dalam pengisian rekam medik secara lengkap, dibutuhkan kepatuhan dari dokter, dokter gigi, atau petugas kesehatan yang mengisi. Terdapat beberapa masalah dalam mengisi rekam medik yaitu seperti pengisian rekam medik yang tidak lengkap, jam kerja yang terlalu pendek sehingga rekam medik tidak diisi, dan banyaknya pasien yang menyebabkan beban kerja dokter semakin berat. Tujuan: Penelitian ini dilakukan untuk mengetahui kelengkapan pengisian rekam medik di klinik unit pelayanan umum RSGM UGM Prof. Soedomo sebagai proksi dari kepatuhan pengisian rekam medik. Metode: Jenis penelitian yang digunakan adalah penelitian deskriptif dengan pendekatan kuantitatif dan menggunakan metode systematic random sampling. Subyek penelitian yang digunakan adalah rekam medik di klinik unit pelayanan umum RSGM UGM Prof. Soedomo pada tahun 2017 sebanyak 120 berkas rekam medik dengan melihat 10 komponen yaitu identitas pasien, tanggal dan waktu, hasil anamnesis, pemeriksaan fisik dan penunjang, odontogram, diagnosis, rencana penatalaksanaan, pengobatan dan atau Tindakan, nama dan tandatangan dokter, serta persetujuan tindakan. Hasil: Hasil penelitian menunjukkan bahwa dari setiap butir-butir komponen yang diteliti tidak lengkap 100%, akan tetapi terdapat butir yang mendekati nilai lengkap 100% yaitu butir rencana perawatan pada komponen rencana penatalaksanaan. Butir pemeriksaan penunjang pada komponen pemeriksaan fisik dan penunjang memiliki nilai kelengkapan terendah. Kesimpulan: Kesimpulan penelitian ini adalah pengisian rekam medik di klinik unit pelayanan umum RSGM UGM Prof. Soedomo tidak lengkap.

ABSTRACT Background: All of healthcare facilities must have records of their patients, the records are in the form of medical record. Medical record must be written completely, because one of the functions of it is as the communication tool between health workers and as a basis of quality assessment of healthcare facilities. In writing medical record completely, compliance of the doctors, dentists, or health workers are needed. There are several problems in filling in medical records such as incomplete filling of medical record, short working hours that makes filling out medical record not done, and the large number of patients which causes over workload of doctors, dentists, or health workers. Objective: This research was conducted to determine the completion of filling out medical records at the General Clinic of RSGM UGM Prof. Soedomo as a proxy of compliance in fulfilling medical records. Method: The type of this research is descriptive study with quantitative approach using systematic random sampling method. The subjects used in this research were 120 medical records at the General Clinic of RSGM UGM Prof. Soedomo in 2017 which assessing 10 components such as identity of the patients, date and time, anamnesis results, physical and laboratory examination, odontogram, diagnosis, treatment planning, treatment, name and signature of the doctors, and informed consent. Results: The results showed that all of items of the components above were not 100% complete, but there was one item close to complete 100% that is treatment planning. Item laboratory examination of physical and laboratory examination components had the lowest completion rate. Conclusion: The conclusion of this study is the fulfillment of medical records at the General Clinic of RSGM UGM Prof. Soedomo in 2017 was not complete.
