Kelengkapan Pengisian Rekam Medik Klinik Orthodonsi RSGM UGM Prof. Soedomo Tahun 2017 Intisari Latar belakang : Rekam Medik adalah berkas yang berisi catatan serta dokumen identitas pasien, hasil pemeriksaan, pengobatan, tindakan, dan pelayanan lain yang telah diberikan kepada pasien. Dampak dari pengisian rekam medik yang tidak lengkap berpengaruh terhadap mutu pelayanan dan patient safety. Tujuan penelitian ini untuk mengetahui kelengkapan dalam pengisian rekam medik klinik orthodonsi tahun 2017. Metode penelitian : Penelitian dilakukan di RSGM UGM Prof. Soedomo. Subjek pada penelitian ini adalah rekam medik orthodonsi RSGM Prof. Soedomo pada tahun 2017. Jumlah sampel berkas rekam medik klinik kepaniteraan orthodonsi yang digunakan sebanyak 114 berkas rekam medik. Penelitian ini dilakukan dengan cara mengumpulkan berkas rekam medik orthodonsi pada tahun 2017, setelah data terkumpul selanjutnya dilakukan pemeriksaan kelengkapan terhadap 8 variabel yaitu: identitas pasien, hasil anamnesis, pemeriksaan objektif, lembar tindakan, lembar kendali perawatan, pemeriksaan penunjang, lembar konsultasi, persetujuan tindakan medik. Kelengkapan pengisian rekam medik diukur menggunakan checklist tabel kerja.. Rekap data menggunakan analisis statistik deskriptif pada Microsoft Excel. Data disajikan berdasarkan persentase sehingga dapat diketahui persentase data yang lengkap, dan data yang tidak lengkap. Hasil penelitian: Hasil penelitian menunjukkan bahwa pengisian berkas rekam medik orthodonsi tahun 2017 di RSGM UGM Prof. Soedomo tidak terisi lengkap. Berkas rekam medik pada identitas pasien terisi 92%, hasil anamnesis 75,4%, pemeriksaan objektif 57,4%, lembar tindakan 99,1%, lembar kendali perawatan 0,9%, pemeriksaan penunjang 8,7%, lembar konsultasi 3,6%, persetujuan tindakan medik 82,5%. Kesimpulan : Kelengkapan pengisian rekam medik orthodonsi tahun 2017 di RSGM UGM Prof. Soedomo rendah. Aitem yang banyak terisi adalah lembar tindakan sebesar 99,1%, sedangkan aitem yang sedikit terisi adalah lembar kendali perawatan sebesar 0,9%. Completeness of Filling the Orthodontic Clinical Medical Record at the RSGM UGM Prof. Soedomo in 2017 Nysa Brilyani, Lisdrianto Hanindriyo, Sri Widiati Abstract Medical record is a file that contains records and documents of the patient���¢�¯�¿�½�¯�¿�½s identity, results of examinations, treatment, management, and other services that have been provided to patients. The impact of filling out an incomplete medical record affects the quality of service and patient safety. The purpose of this study was to determine the completeness of filling the orthodontic clinical medical record in 2017. The study was conducted at RSGM UGM Prof. Soedomo. The subjects in this study were the orthodontic medical records of RSGM UGM Prof. Soedomo in 2017. The total number of orthodontic clerical clinic medical record file used was 114 medical record files. This research was conducted by collecting orthodontic medical records in 2017, followed by a complete examination completeness of the 9 variables, namely : patient identity, results of history taking, objective examination, action sheets, care control, supporting examination, consultation sheets, approval of medical measures. Completeness of filling medical records was verify using a work Tabel checklist. Data recap using descriptive statistical analysis on Microsoft Excel. Presentation of data from the results of this study is displayed in tabular and text form. Data is presented based in a precentage so that it can be seen the precentage of complete data, and incomplete data. The results showed that filling the orthodontic medical record file in 2017 at RSGM UGM Prof. Soedomo was not completely filled. The medical record file on the patient's identity was 92% filled, the results of the history were 75.4%, objective examination 57.4%, action sheet 99.1%, care examination sheet 0.9%, supporting examination 8.7%, consultation sheet 3, 6%, approval of medical treatment 82.5%. The conclusion completeness of filling orthodontic medical records of RSGM UGM Prof. Soedomo in 2017 is low. The most filled item is the action sheet at 99.1%, while the least filled item is the maintenance control sheet at 0.9%.
