KEPATUHAN PENGISIAN REKAM MEDIS DI KLINIK ILMU KEDOKTERAN GIGI ANAK (IKGA) RSGM PROF. SOEDOMO PADA TAHUN 2017

Semua pelayanan kesehatan harus mempunyai catatan mengenai pasien berupa rekam medis, sehingga dalam penulisan rekam medis harus lengkap dan agar rekam medis dapat terisi dengan lengkap harus ada kepatuhan dari orang yang bertanggung jawab dalam menulis rekam medis. Dalam pengisian rekam medis terdapat beberapa masalah seperti pengisian rekam medis yang tidak lengkap, penulisan rekam medis yang tidak spesifik, waktu yang sangat terbatas, jumlah pasien yang banyak, dan terkadang pada penulisan rekam medis juga terburu-buru. Oleh karena itu, penelitian ini dilakukan untuk mengetahui kepatuhan pengisian rekam medis di klinik Ilmu Kedokteran Gigi Anak (IKGA) RSGM Prof. Soedomo pada tahun 2017. Jenis penelitian yang digunakan adalah penelitian deskriptif dengan pendekatan kuantitatif yang menggunakan data sekunder yaitu data rekam medis yang ada di klinik Ilmu Kedokteran Gigi Anak (IKGA) RSGM Prof. Soedomo pada tahun 2017 dengan menilai 10 butir variabel pada rekam medis. Teknik pengambilan sampel yang digunakan adalah simple random sampling. Hasil penelitian menunjukkan bahwa hanya beberapa butir seperti anamnesis pada keluhan utama, riwayat kesehatan keluarga (gigi dan umum), riwayat kesehatan umum, riwayat kesehatan keluarga, dan pencegahan penyakit gigi (menyikat gigi, topikal aplikasi fluor, tablet fluor, dan kumur-kumur), pemeriksaan fisik pada peninjauan sistem tubuh serta odontogram yang memiliki kelengkapan 100%, sementara butir lain tidak mencapai 100%. Kesimpulan penelitian ini adalah pengisian rekam medis pada klinik Ilmu Kedokteran Gigi Anak (IKGA) RSGM Prof. Soedomo tidak patuh.

All of health services must have a record of the patient in the form of a medical record, so in writing medical records must be complete and for medical records to be filled completely there must be compliance from the person responsible for writing medical records. In filling the medical record there are several problems such as incomplete filling medical records, not specific in writing, a very limited time, large number of patients, and sometimes hurrying in filling it. Therefore, this study was conducted to determine compliance with filling the medical records at the Pediatric Dentistry clinic of RSGM Prof. Soedomo in 2017. The type of this research is descriptive study with a quantitative approach that uses files medical record in the Pediatric Dentistry clinic of RSGM Prof. Soedomo in 2017 as a secondary data by assessing 10 items in the medical record. The sampling technique used is simple random sampling. The results showed that only a few items such as history taking in chief complaint, family health history (dental and general), general health history, family health history, and prevention of dental disease (tooth brushing, topical application of fluoride, fluoride tablets, and mouth rinses), physical examination in review of body system and odontogram that were 100% completeness, meanwhile the other items are not 100% filled. The conclusion of this study is medical records filling in Pediatric Dentistry clinics of RSGM Prof. Soedomo is not compliant.
