PERANCANGAN MAP, FORMULIR RAWAT JALAN, DAN FORMULIR ASUHAN KEBIDANAN DENGAN SOP PENGISIAN DI PUSKESMAS CIPERNA KABUPATEN CIREBON

Latar Belakang : Puskesmas Ciperna mempunyai wacana melakukan pengadaan terkait map rekam medis dan memperbaiki formulir rawat jalan serta asuhan kebidanan, karena berpengaruh terhadap akreditasi yang akan dilakukan pada tahun 2021. Puskesmas Ciperna belum pernah menerapkan map rekam medis yang mengakibatkan formulir hanya dijadikan satu menggunakan staples dan berpengaruh terhadap mutu dari rekam medis tersebut. Disamping itu, formulir rawat jalan dan asuhan kebidanan yang dipakai masih terdapat kekurangan jika dilihat dari sisi pengguna dan dilihat dari 3 aspek yaitu anatomi, isi dan fisik. Tujuan : Merancang map rekam medis, merancang ulang formulir rawat jalan, formulir asuhan kebidanan dan SOP yang disesuaikan dengan formulir dan map yang telah terpilih. Metode : Metode perancangan map dan formulir ini mengacu pada metode dan tahapan RCH (Royal Child Hospital). Hasil : Diperoleh hasil 2 alternatif rancangan pada map rekam medis, formulir rawat Jalan dan formulir asuhan kebidanan. Pada proses pemilihan map rekam medis terpilihlah map rekam medis alternatif 2, Formulir rawat jalan alternatif 1 dan formulir asuhan kebidanan alternatif 2. Serta rancangan SOP disesuaikan dengan formulir dan map yang telah terpilih. Kesimpulan : Dihasilkan rancangan baru map rekam medis, rancangan ulang formulir rawat jalan dan asuhan kebidanan yang disesuaikan dengan aspek isi, anatomi dan fisik dengan mempertimbangkan kebutuhan pengguna. Serta rancangan SOP pengisian formulir dan map yang telah terpilih. Kata Kunci : Map Rekam Medis, Formulir Rawat Jalan, Formulir Asuhan Kebidanan, Standar Prosedur Operasional.

Background: Puskesmas Ciperna has discourse to procure related medical record folders and improve outpatient forms and midwifery care because it affects the accreditation that will be done in 2021. Puskesmas Ciperna has never applied a medical record folder wich resulted the formulir that just made one by using staples and effect on the quality of medical record. Besides that, outpatient and midwifery care forms that being used is still deficiency when seen on users side and three aspects such as anatomy, content and physical. Objective: The Method of designing folders and redesigning forms refers to the methods and stages of the RCH Forms Committee form development and SOP is adapted to the selected forms and folders. Result: Two alternative design for folders and outpatient forms and midwifery care forms. In the process of selecting the medical record folder, the alternative medical record folder 2 is selected, the alternative outpatient form 1 and the alternative midwifery care form 2. As well as the draft SOP according to the selected forms and folders. Conclusion: A new medical record folder design was produced, a redesign of the outpatient and midwifery forms which is adapted to the content, anatomy, and physical aspects by considering the users needs.
