ANALISIS FAKTOR PENYEBAB KETIDAKLENGKAPAN PENDOKUMENTASIAN RESUME PASIEN PULANG PADA PASIEN JKN DI RSKIA PKU MUHAMMADIYAH KOTAGEDE YOGYAKARTA

Latar Belakang: Resume pasien pulang merupakan lembaran yang berisi ringkasan pelayanan yang diberikan selama pasien dirawat hingga pasien dipulangkan baik dalam keadaan hidup maupun meninggal. Ketidaklengkapan pendokumentasian resume pasien JKN mengakibatkan klaim tertunda dan ditangguhkan hingga bulan berikutnya. Berdasarkan hasil studi pendahuluan, didapatkan persentase ketidakterisian resume sebesar 13,33% pada komponen identifikasi, 77,62% pada komponen laporan penting, dan 58,67% pada komponen autentikasi. Tujuan: Menganalisis faktor penyebab dan dampak ketidaklengkapan pendokumentasian lembar resume pasien pulang pada pasien JKN di RSKIA PKU Muhammadiyah Kotagede Yogyakarta. Metode: Jenis penelitian kualitatif dengan rancangan penelitian studi kasus. Subjek dalam penelitian ini terdiri dari 5 orang responden. Objek dalam penelitian ini adalah formulir resume pasien pulang pada pasien JKN. Teknik pengambilan data menggunakan wawancara, observasi, dan studi dokumentasi. Uji keabsahan data menggunakan triangulasi sumber dan triangulasi teknik. Hasil: Resume hanya boleh diisi oleh DPJP yang berkaitan, resume yang belum lengkap selalu dimintakan kembali kelengkapannya kepada DPJP. Faktor penyebab ketidaklengkapan resume pasien JKN adalah keterbatasan waktu DPJP, belum dilaksanakannya reward and punishment, penggunaan SPO yang belum optimal, dan belum diberlakukannya sosialisasi secara rutin. Resume yang belum lengkap berdampak pada terputusnya kesinambungan informasi riwayat kesehatan pasien, penundaan klaim BPJS, dan menambah beban kerja petugas klaim BPJS. Dalam rangka mengatasi ketidaklengkapan resume pasien JKN, pihak rumah sakit telah melakukan beberapa upaya. Kesimpulan: Ketidaklengkapan pendokumentasian resume pasien pulang pada pasien JKN yang disebabkan oleh beberapa faktor dapat mengakibatkan penundaan klaim BPJS (Claim Pending). Background: Discharge Summary is a form that contains a summary of services that have been provided during the hospitalization time until the patient is discharged both alive and dead. The incompleteness documentation of the discharge summary, especially for NHI patients resulting in claims being delayed and suspended until the following month. Based on the results of the preliminary study, the percentage of discharge summary incompleteness was 13.33% in the identification, 77.62% in the important report, and 58.67% in the authentication. Objective: To analyze the cause factors and impacts of incompleteness documentation of NHI patient's discharge summary at RSKIA PKU Muhammadiyah Kotagede Yogyakarta. Methods: The type of this research was qualitative research with with a case study research design. The subjects of this research consisted of 5 respondents. The object of this research was NHI patient's discharge summary form. The data was taken by interview, observation and documentation studies. Data validity test used source and technic triangulation. Results: The discharge summary form may only be filled out by physician, the incomplete resume was always returned for completeness to the physician. Factors causing incompleteness documentation of NHI patient's discharge summary were the limited time and busyness of the physician, the reward and punishment policy was not been implemented, the use of SOP that has not been optimal, and not yet enforced routine socialization. The impact of incompleteness documentation of NHI patient's discharge summary were the discontinuation about patient's medical history, delayed insurance claims, and increased the workload of insurance claim officers. In order to overcome the incompleteness, the hospital has made several efforts. Conclusion: The incomplete documentation of NHI patient's discharge summary that were caused by several factors can result in delayed insurance claims (Claim Pending).
