THE EFFECT OF LEUKODEPLETION ON LEUKOCYTE COUNT OF PACKED RED CELLS (PRC) AT UNIT PELAYANAN TRANSFUSI DARAH RSUP DR. SARDJITO YOGYAKARTA

Latar Belakang. Leukodeplesi adalah proses untuk mengeluarkan sel darah putih (WBC) dari komponen darah sebelum ditransfusikan ke penerima. Metode ini diambil oleh bank darah untuk memastikan tidak ada efek buruk pada transfusi. Leukodeplesi itu sendiri mencegah penerima untuk efek samping imunologis. Munculnya WBC dalam darah dapat menyebabkan efek buruk pada penerima, seperti reaksi alergi, reaksi demam, dan refraktilitas trombosit karena alloimunisasi,. Penggunaan leukodeplesi saat ini sangat penting untuk mengimbangi seringnya pasien yang ditransfusikan tinggi. Donor darah tidak hanya digunakan untuk transfusi, tetapi juga untuk donor organ. Objektif. Tujuan dari penelitian ini adalah untuk menganalisa jumlah sel darah putih sebelum dan sesudah leukodeplesi Metode. Penelitian ini menggunakan studi cross sectional untuk membedakan hasil dari teknik filtrasi leukodepletion di RSUP Sardjito. Hasil utama akan membedakan jumlah leukosit, sebelum dan sesudah leukodeplesi menggunakan teknik filtrasi. Semua data yang digunakan dalam penelitian ini diperoleh dari data komponen donor darah di RSUP Sardjito, Yogyakarta. Hasil. Dari lima puluh tujuh (57) sampel pada pre-leukodeplesi, terdapat lima puluh enam (56) sampel yang mencapai angka diatas nilai standar non-leukodeplesi PRC dan satu (1) sampel yang terdapat dalam nilai standar. Setelah dilakukan leukodeplesi terdapat dua puluh dua (22) sampel yang berada di nilai standar dan tiga puluh lima (35) sampel yang berada diatas nilai normal. Namun semua sampel mengalami penurunan signifikan pada nilai jumlah leukosit. Median dari pre-leukodeplesi nilai leukosit adalah 13.23 x 103/uL dan 0.03 x 103/uL untuk post-leukodeplesi. Kesimpulan. Terdapat penurunan jumlah leukosit sebelum dan sesudah di leukodeplesi. Penurunan jumlah leukosit secara statistik signifikan. Kata kunci. leukodeplesi, filtrasi, leukosit

Background. Leukodepletion is the process to remove the white blood cells (WBC) from the blood component before it transfused into the recipients. This method taken by the blood bank to make sure there was no adverse effect on the transfusion. The leukodepletion itself prevent the recipients for immunologic adverse effect. The appearance of WBC in blood can caused adverse effects in recipient, such as allergic reactions, febrile reactions, platelet refractoriness due to alloimmunization, etc. The use of leukodepletion in nowadays was very important to compensate the frequent of transfused patients is high. The blood donor was not only use for transfusion, but also for organ donor. Objective. The purpose of this study is to analyze of leukocyte count before and after leukodepletion. Method. This study used the cross sectional study to differentiate of the outcome from the filtration technique of leukodepletion in RSUP Sardjito. The primary outcome will distinguish the amount of leukocyte, before and after leukodepletion using filtration technique. All the data used in this study was obtained from the data of the blood donor component in RSUP Sardjito, Yogyakarta. Results. From the fifty-seven (57) samples in pre-leukodepletion, there were fifty-six (56) samples that reached figures above the reference range non-leukodepletion PRC value and one (1) sample contained in the reference range. After leukodepletion there were twenty-two (22) samples that were at normal values and thirty-five (35) samples that were above normal values. However, all samples experienced a significant decrease in the value of the leukocyte count. The median of pre-leukodepletion of leukocyte count is 13.23 x 103/uL and 0.03 x 103/uL for post-leukodepletion. Conclusion. There was a decreasing on leukocyte count between pre and post-leucodepleted PRC. The decrease of leukocyte count was statistically significant. Keywords. leucodepletion, filtration, leukocyte
