THE PATTERN OF CELLULAR INTERACTION USING SPECTROPHOTOMETRY AND WESTERGREN METHOD OF EDTA- BLOOD IN BREAST CANCER PATIENT: INFLUENCE OF NEUTROPHILS

2020 | Skripsi | S1 KEDOKTERANLatar Belakang: Kanker payudara adalah pertumbuhan sel-sel payudara yang tidak terkontrol dan merupakan salah satu keganasan umum yang terjadi pada wanita. Di rumah sakit, diagnosis kanker payudara dikonfirmasi oleh beberapa pendekatan yang mencakup biopsi, tetapi invasif dan dianggap terlambat. Diagnosis alternatif oleh protein biomarker masih dianggap terlambat untuk proses isolasi panjang sementara kanker sedang berkembang. Pengukuran laju sedimentasi eritrosit (ESR) dengan metode Westgren (dengan mengukur batas zona phlogistic (Bpz) selama 1 jam) kurang spesifik sebagai alat skrining meskipun dapat ditingkatkan dengan metode spektrofotometri. Penelitian ini bertujuan untuk mengetahui pengaruh jumlah neutrofil pada absorbansi dan pola BPZ pada EDTA-darah pasien kanker payudara. Metode: Sampel darah vena diambil dari subyek normal (sehat) dan kanker payudara di Rumah Sakit Sardjito dan Laboratorium Patologi Klinik Fakultas Kedokteran, Kesehatan Masyarakat dan Keperawatan UGM. Tes darah rutin dilakukan di Laboratorium Patologi Klinik Rumah Sakit Sardjito dan Fakultas Kedokteran, Kesehatan dan Keperawatan Masyarakat UGM, sedangkan data absorbansi dari metode spektrofotometri dan Bpz dari metode Westergren dilakukan di Pusat Penelitian Bioteknologi UGM. Independent t-test dan Mann Whitney dilakukan untuk membandingkan parameter deterministik (D) dan stochastic (S) dari absorbansi dan pola Bpz subyek normal dan kanker payudara, yang membentuk fungsi kuadratik. Korelasi Pearson dan Spearman dilakukan untuk menentukan pengaruh jumlah neutrofil pada parameter deterministik dan stokastik dari pola absorbansi dan pola Bpz. Hasil: Jumlah neutrofil pada subyek kanker payudara (3,605 (50 - 23,615) lebih rendah dari subyek normal (4,286 (1,195-9,828). Pola absorbansi antara subyek kanker payudara dan normal berbeda pada parameter S1 (P <0,005) dan gula aren) berbeda pada parameter D1, D2, D3, S2, dan S3 (P> 0,05) Pola Bpz antara subyek kanker payudara dan normal berbeda pada parameter D1, D2, D3, S1, dan S2 (P <0,05), sedangkan Parameter S3 tidak normal (P> 0,05) Pada subyek kanker payudara, pengaruh jumlah neutrofil dalam pola absorbansi rendah (r = 0,20 - 0,399), sedangkan pada pola Bpz pengaruh jumlah neutrofil sangat rendah (0,00 - 0,199 ). Background: Breast cancer is uncontrolled growth of breast cell and is one of the common malignancies that happened in women. At the hospital diagnosis of breast cancer is confirmed by several approach which include biopsy, but it is invasive and considered late. Alternative diagnosis by biomarker proteins is still considered late for its long isolation process while the cancer is progressing. Measurement of erythrocyte sedimentation rate (ESR) by Westergren method (by measuring boundary of phlogistic zone (Bpz) for 1 hour) were less specific as a screening tool although it can be improved by spectrophotometric method. This study aims to determine the influence of neutrophil counts to the absorbance and Bpz pattern in EDTA-blood of breast cancer patients. Method: Venous blood samples were taken from normal (healthy) and breast cancer subjects at Sardjito Hospital and Clinical Pathology Laboratory of Faculty of Medicine, Public Health and Nursing UGM. Routine blood tests were conducted at the Clinical Pathology Laboratory of Sardjito Hospital and Faculty of Medicine, Public Health and Nursing UGM, while the absorbance data from spectrophotometric method and the Bpz of Westergren method were carried out at Research Center for Biotechnology PAU Building UGM. Independent t-test and Mann Whitney test were performed to compare the deterministic (D) and stochastic (S) parameters of the absorbance and Bpz patterns of normal and breast cancer subjects, which form a quadratic function. Pearson and Spearman correlation were performed to determine the influence of neutrophil counts on deterministic and stochastic parameters of the absorbance and Bpz patterns. Result: Neutrophil counts on breast cancer subjects (3,605(50 - 23,615) was lower than the normal subjects (4,286(1,195-9,828). Pattern of absorbance between normal and breast cancer subjects was different in S1 parameter (P<0.005) and weren't different on D1, D2, D3, S2, and S3 parameters (P>0.05). The Bpz patterns between normal and breast cancer subjects were different on D1, D2, D3, S1, and S2 parameters (P<0.05), while S3 parameter wasn't normal (P>0.05). In breast cancer subjects, the correlation of neutrophil counts in absorbance pattern was low (r=0,20-0,399), while on Bpz pattern the correlation of neutrophil count was very low (0,00-0,199). Conclusion: There are differences of absorbance and Bpz pattern of EDTA-blood between normal and breast cancer subjects. In breast cancer subjects the effect of neutrophil count to absorbance pattern is low
