Stroke, despite all the diagnostic and management improvements, is still considered a high burden disease worldwide. Stroke has been actively increasing Disability Adjusted Life Years (DALY) and deaths compared to all diseases since the last 2 decades, especially in Indonesia, where it is named as the second highest cause of disability and mortality combined. Stroke may be present with varying degree of fatality and symptoms, and all is equally crucial to be detected and treated as early as possible to decrease the probability of permanent neurological deficits. NIHSS has been widely used as an assessment tool for functional outcome, and its possible function to differentiate the location of lesion is still scarcely known. Therefore, it is important to understand the NIHSS scoring system and the location of infarction, because it may able to reduce the urgency of going through MRI and CT-Scan.
