PERANCANGAN STANDAR OPERASIONAL PROSEDUR (SOP) PENYUSUTAN DAN PEMUSNAHAN BERKAS REKAM MEDIS DI PUSKESMAS JATINOM KLATEN

Latar Belakang: Terjadi kepadatan rak penyimpanan di ruang filling rekam medis di Puskesmas Jatinom. Puskesmas terkendala untuk melaksanakan penyusutan dan pemusnahan bekas rekam medis. Dikarenakan Puskesmas belum memiliki SOP penyusutan dan pemusnahan berkas rekam medis. Selain itu, Puskesmas akan melaksanakan reakreditasi dimana dalam salah satu elemen penilaiannya mengharuskan adanya SOP dalam setiap kegiatan penyimpanan berkas. Tujuan: Merancang SOP kegiatan penyusutan dan pemusnahan berkas rekam medis yang akan diakukan di Unit Rekam Medis Puskesmas Jatinom Klaten. Metode Penelitian: Metode pengambilan data dilakukan dengan cara wawancara, observasi dan studi dokumentasi. Subjek dalam perancangan ini adalah petugas rekam medis dan dokter klinik umum Puskesmas Jatinom. Objek dalam perancangan ini adalah penyusutan dan pemusnahan berkas rekam medis di Puskesmas Jatinom. Hasil: Rancangan SOP penyusutan dan pemusnahan berkas rekam medis sesuai dengan kebutuhan Puskesmas Jatinom. Format SOP yang digunakan sesuai pada Pedoman Penyusunan Dokumen Akreditasi FKTP Puskesmas Jatinom dengan prosedur penyusutan dan pemusnahan sesuai menggunakan tata cara penyusutan menurut Depkes RI tahun 2006 dan pemusnahan menurut Dirjen Yanmed tahun 1995. Kesimpulan: Penyusunan SOP penyusutan dan pemusnahan berkas rekam medis dilakukan berdasarkan format dan sesuai identifikasi kebutuhan di Puskesmas Jatinom. Rancangan Standar Operasional Prosedur (SOP) penyusutan dan pemusnahan berkas rekam medis dapat disahkan oleh Kepala Puskesmas Jatinom sehingga dapat menjadi pedoman pelaksanaan penyusutan dan pemusnahan berkas rekam medis.

Objective: Designing an SOP for the activity of shrinking and destroying the medical record file which will be carried out at the Medical Record Unit of the Puskesmas Jatinom in Klaten. Methods: The method of collecting data is done by interview, observation and documentation study. The subject in this design was the medical record officer and general clinic doctor at Puskesmas Jatinom. The object in this design is the shrinking and destruction of the medical record file at Puskesmas Jatinom. Results: The draft SOP for depreciation and destruction of medical record files is in accordance with the needs of Puskesmas Jatinom. The SOP format used is in accordance with the Guidelines for Preparing the Accreditation Documents for FKTP Jatinom Puskesmas with depreciation and culling procedures in accordance with the procedures for depreciation according to Depkes RI in 2006 and annihilation according to Dirjen Yanmed in 1995. Conclusion: Preparation of SOP depreciation and destruction of medical record files is carried out based on the format and according to the identification of needs in the Puskesmas Jatinom. The draft Standard Operating Procedure (SOP) for shrinking and destroying the medical record file can be endorsed by the Head of Puskesmas Jatinom so that it can become a guideline for the implementation of the shrinking and destruction of the medical record file.
