PERANCANGAN ULANG FORMULIR GIGI DAN MAP REKAM MEDIS DI KLINIK GIGI R+ CONDONGCATUR YOGYAKARTA

Latar Belakang: Rekam medis adalah catatan yang berisi informasi kondisi pasien dan pelayanan yang telah diberikan. Klinik Gigi R+ Condongcatur telah melaksanakan pendokumentasian rekam medis pasien menggunakan formulir. Formulir gigi yang digunakan mempunyai kekurangan dari aspek formulir dan item minimal pada rekam medis klinik gigi. Map rekam medis yang digunakan berbahan tipis yaitu kertas manila dan ukurannya lebih kecil dibandingkan formulir sehingga mudah robek dan tidak dapat melindungi formulir seutuhnya. Map rekam medis memiliki kekurangan item. Berdasarkan hal tersebut perancang mempunyai ide berupa membuat rancangan ulang formulir gigi dan map rekam medis sesuai dengan kebutuhan dan standar. Tujuan: Merancang ulang formulir gigi dan map rekam medis sesuai dengan standar dan kebutuhan Klinik Gigi R+ Condongcatur. Metode: Perancangan formulir gigi dan map rekam medis ini mengacu pada tahapan pengembangan formulir RCH Forms Committee (2006). Waktu perancangan dimulai dari Desember 2019 hingga Maret 2020. Subjek perancangan adalah dokter gigi, perawat gigi, dan petugas rekam medis. Objek perancangan yakni formulir gigi dan map rekam medis. Hasil: Penelitian ini menghasilkan 2 alternatif formulir gigi dan map rekam medis. Berdasarkan hasil pemilihan, formulir gigi alternatif 1 dan map rekam medis alternatif 2 dipilih oleh responden. Rancangan yang terpilih selanjutnya diuji coba dan diperbaiki sesuai dengan saran pengguna. Kesimpulan: Rancangan baru formulir gigi dan map rekam medis dihasilkan sesuai dengan standar dan kebutuhan pengguna. Sebaiknya Klinik Gigi R+ Condongcatur menerapkan rancangan dan pembaruan standar operasional prosedur terkait penggunaannya. Kata Kunci: Formulir gigi, map rekam medis, perancangan ulang.

Background : Medical record is a record that contains information about patient's condition and services that have been provided. Dental Clinic R+ Condongcatur has documented medical records using form. Dental form used has lack of form aspects and minimal items. Medical record folder used is lack of item, less thick and it size is smaller than form size, it’s easily torn and can’t protect the form. therefore, the designer has the idea to redesign dental form and medical record folder according to the needs of the user and the standards. Objective: Redesigning dental form and medical record folder according to appropriate with the standard and to the needs of Dental Clinic R+ Condongcatur. Methods: Design of dental form and medical record folder refered to stages of RCH Forms Committee. It was started from December 2019 until March 2020. The subjects were dentists, dental nurses, and medical recorder. The objects were the dental form and medical record folder. Results : This research produced 2 alternative dental forms and medical record folders. Based on the selection, dental form alternative 1 and medical record folder alternative 2 were selected by respondents. The selected design is then tested and corrected according to the user's suggestion. Conclusion : New draft of dental form and medical record folder is produced according to the standards and the needs. It’s better for Dental Clinic R+ Condongcatur to implement the design and updating the standard operational procedure related to its use. Keywords : Dental form, medical record folder, redesign.
