PENGARUH APLIKASI GEL EKSTRAK KULIT Citrus sinensis 10% PASCA KURETASE TERHADAP KEPADATAN SERABUT KOLAGEN TULANG ALVEOLAR PADA PROSES PENYEMBUHAN PERIODONTITIS Rattus norvegicus

Periodontitis merupakan radang jaringan pendukung gigi yang menyebabkan kerusakan kronis. Ciri-ciri patologis periodontitis adalah tipisnya serabut kolagen. Kuretase adalah perawatan yang berfungsi untuk menghilangkan jaringan yang mengalami inflamasi pada periodontitis. Pemberian bahan terapi tambahan dapat mendukung penyembuhan periodontitis. Aplikasi gel ekstrak kulit Citrus sinensis 10% pasca kuretase dapat meningkatkan produksi reseptor insulin-like growth factor-1 (IGF-1) yang menstimulasi sintesis kolagen. Penelitian ini bertujuan untuk mengetahui pengaruh aplikasi gel ekstrak kulit Citrus sinensis 10% pasca kuretase terhadap kepadatan serabut kolagen tulang alveolar pada proses penyembuhan periodontitis Rattus norvegicus. Delapan belas Rattus norvegicus dibagi menjadi 3 kelompok yaitu kontrol negatif, kelompok perlakuan, dan kontrol positif. Gigi insisivus sentralis rahang bawah Rattus diligasi menggunakan silk ligature 3.0 selama 7 hari untuk menginduksi periodontitis. Kuretase dilakukan pada dinding poket yang terbentuk. Poket kelompok kontrol negatif diaplikasikan CMC-Na 2%, kelompok perlakuan diaplikasikan ekstrak kulit Citrus sinensis 10%, dan kelompok kontrol positif diaplikasikan asam hialuronat 0.2% (gel Gengigel®). Rattus dikorbankan 2 ekor pada hari ke-3, 7, dan 14. Sediaan histologis dibuat dengan pengecatan Trichrom Mallory dan diamati menggunakan mikroskop cahaya yang dilengkapi dengan OptiLab Viewer. Analisis hasil penelitian menggunakan uji ANAVA dua jalur dan uji Post Hoc metode LSD. Hasil analisis data menunjukkan perbedaan kepadatan serabut kolagen yang bermakna (p<0,05) antara kontrol negatif terhadap perlakuan dan kontrol positif pada hari ke-7, dan 14. Kesimpulan penelitian ini adalah aplikasi gel ekstrak kulit Citrus sinensis 10% berpengaruh meningkatkan kepadatan serabut kolagen tulang alveolar pada proses penyembuhan periodontitis Rattus norvegicus.

Periodontitis is an inflammation of the supporting tissues of the teeth that cause chronic damage. The pathological features of periodontitis are the thinness of collagen fibers. Curettage is a treatment to remove inflamed tissue in periodontitis. Providing additional therapeutic materials can support the healing of periodontitis. Application of 10% Citrus sinensis peel extract gel post-curettage can increase the production of insulin-like growth factor-1 (IGF-1) receptors which stimulate collagen synthesis. This study aims to determine the effect of 10% Citrus sinensis peel extract gel post-curettage on the density of alveolar bone collagen fibers in the healing process of Rattus norvegicus periodontitis. Eighteen Rattus norvegicus were divided into 3 groups: negative control, treatment group, and positive control. Mandibular central incisors of the Rattus were ligated using silk ligature 3.0 for 7 days to induce periodontitis then curettage is done on the pocket wall. Negative control group was applied 2% CMC-Na, the treatment group was applied 10% Citrus sinensis peel extract, and the positive control group was applied 0.2% hyaluronic acid (Gengigel®) in the pocket. Two Rattus was sacrificed on day 3, 7, and 14. Histological preparations were painted by Trichrom Mallory and observed using a light microscope equipped with OptiLab Viewer. Analysis of the results of research using the two-way ANAVA test and Post Hoc test LSD method. The results showed there were significant differences in collagen fiber density (p <0.05) between negative controls towards treatments and positive controls on days 7, and 14. The conclusion of this study is the application of 10% Citrus sinensis peel extract gel has an effect on increasing the density of alveolar bone collagen fibers in the healing process of Rattus norvegicus periodontitis.
