COST OF ILLNESS PADA PASIEN DIABETES RETINOPATI DI RUMAH SAKIT DI YOGYAKARTA

Diabetes retinopati merupakan salah satu komplikasi mikrovaskuler tertinggi pada diabetes melitus (DM) tipe 1 maupun 2. Penyakit ini membutuhkan rentang waktu pengobatan yang panjang sehingga membutuhkan biaya yang besar. Dalam studi Cost of Illness (COI) dilakukan pengukuran beban ekonomi dari suatu penyakit pada masyarakat. Penelitian ini bertujuan untuk mengetahui biaya penyakit diabetes retinopati berdasarkan perspektif societal dan mengetahui faktor faktor yang mempengaruhi besarnya biaya penyakit diabetes retinopati. Jenis penelitian ini merupakan penelitian analitik observasional dengan rancangan penelitian cross sectional. Subjek penelitian adalah pasien diabetes retinopati yang berkunjung ke rumah sakit pada periode November 2018 sampai Januari 2019. Data diambil secara retrospektif untuk biaya medik langsung, concurrent untuk biaya non medik langsung dan biaya tidak langsung. Data untuk biaya medik langsung diperoleh dari bagian keuangan RSUP Dr. Sardjito dan RS Dr. Yap Yogyakarta. Untuk biaya non medik langsung dan biaya tidak langsung diperoleh langsung dari pasien dengan menggunakan kuesioner. Analisis data dilakukan dengan uji Mann Whitney dan Kruskal Wallis untuk menganalisis faktor-faktor yang mempengaruhi biaya. Total biaya penyakit diabetes retinopati pada 67 pasien di RSUP Dr. Sardjito adalah Rp 401.502.100 dengan rata rata per pasien Rp 5.992.569 �± Rp 4.880.578 dengan persentase biaya medik langsung 83,82% , biaya non medik langsung 15,13% dan biaya tidak langsung 1,05% dari total biaya. Total biaya penyakit pasien diabetes retinopati pada 60 pasien di RS Dr. Yap adalah Rp 167.101.950 dengan rata rata per pasien Rp 2.789.033 �± Rp 2.847.036 dengan persentase biaya medik langsung 67,45% , biaya non medik langsung 29,92% dan biaya tidak langsung 2,63% dari total biaya. Frekuensi kunjungan dan jenis terapi memberikan pengaruh (p=0,000) terhadap biaya medik langsung di RSUP Dr. Sardjito dan RS Dr. Yap. Lama menderita DR memberikan pengaruh terhadap biaya medik langsung di RS Dr. Yap. Komplikasi memberikan pengaruh terhadap biaya medik langsung di RSUP Dr. Sardjito. status bekerja memberikan pengaruh terhadap biaya tidak langsung pada pasien diabetes retinopati rawat jalan di RSUP Dr. Sardjito dan RS Dr. Yap

Diabetic retinopathy is one of the highest microvascular complications in type 1 and 2 diabetes mellitus. The disease requires a long treatment period, it requires a large cost. Cost of illness is carried out measuring the economic burden of disease in society. This study aims to determine the cost of illness diabetic retinopathy based on the societal perspective and find out the factors that influence the cost of diabetic retinopathy. This research is an observational analytic with a cross sectional study design. The research subjects are patients with type 1 or 2 diabetes mellitus with diagnose diabetic retinopathy in the period November 2018 until January 2019. Data for direct medical cost taken from the department finance at Sardjito hospital and Yap . Direct non medical cost and indirect cost obtained directly from patient using a questionnaire. Data analysis was performed by Mann Whitney test and Kruskal Wallis to analyze the factors that influence cost. The total cost of diseases of diabetic retinopathy with 67 patients in Sardjito hospital was Rp 401.502.100 with an average of Rp 5.992.569 �± Rp 4.880.578 with a percentage of direct medical costs 83.82%, direct non-medical costs 15.13% and indirect costs 1.05% of total costs. The total cost of disease for patients with diabetes retinopathy with 60 patients in Yap hospital was Rp. 167.101.950 with an average of Rp. 2.789.033 �± Rp. 2.847.036 with the percentage of direct medical costs 67.45%, direct non-medical costs 29.92% and indirect costs 2.63% of total costs. The visit frequency and type of therapy had an effect (p = 0,000) on direct medical costs in Sardjito and Yap hospital. Duration of diabetic retinopathy has an effect on direct medical cost in Yap hospital. Complication has an effect on direct medical cost in Sardjito hospital. Job status has an effect on indirect cost in outpatient diabetic retinopathy patients at Sardjito and Yap hospital
